Estring Vaginal Ring information

Estring Vaginal Ring is an estrogen replacement therapy (ERT) used to treat moderate to severe symptoms of postmenopausal vulvar and vaginal atrophy, such as vaginal dryness, burning, itching, and painful intercourse. It works by releasing a continuous, low dose of estradiol directly into the vaginal tissues, helping to restore the natural lining of the vagina and replenish local estrogen levels that decline after menopause.

Side Effects

Commonly reported side effects include an increase in vaginal discharge, vaginal discomfort, back pain, headache, and abdominal pain. You may also experience nausea, breast tenderness, flu-like symptoms, or localized genital itching and irritation.

Contact your doctor if you have any of the serious side effects, including:

• Cardiovascular Events: Although systemic absorption is low, there is a potential risk of blood clots (venous thromboembolism), stroke, or heart attack associated with estrogen therapy.

• Endometrial Hyperplasia: An increased risk of cancer of the uterine lining if used without a progestogen in women who still have a uterus.

• Toxic Shock Syndrome (TSS): A rare but serious bacterial infection linked to the long-term presence of a vaginal ring. Symptoms include sudden high fever, faintness, or a sunburn-like rash.

• Vaginal Erosion or Ulceration: Potential for the ring to cause tissue irritation, ulceration, or embedding within the vaginal wall.

Seek medical attention immediately if you experience sudden leg pain or swelling, severe chest pain, sudden shortness of breath, a severe or unusual headache, sudden vision changes, or a new lump in your breast. You should also seek urgent care if you develop signs of toxic shock syndrome, such as a sudden high fever, vomiting, diarrhea, or dizziness upon standing. Do not remove or stop using this medication without consulting your doctor if it is part of a prescribed regimen, as your vaginal symptoms may quickly return.

Precautions:

Do not use Estring Vaginal Ring if you have a known hypersensitivity to estradiol or any of the silicone polymers or inactive components used to manufacture the ring. Tell your doctor about your medical history, especially if you have:
o Active or past blood clots (such as deep vein thrombosis or pulmonary embolism).
o Known or suspected pregnancy.
o Known, suspected, or history of breast cancer or other estrogen-dependent cancers.
o Narrow vagina, vaginal prolapse, or conditions that make the vagina susceptible to severe irritation or ulceration.
o Recent heart attack, stroke, or severe liver disease.
o Undiagnosed abnormal vaginal bleeding.

Because Estring works primarily locally, significant interactions with oral medications are highly unlikely. However, it may interact with other vaginal preparations (such as antifungal creams or douches), which can affect the release or absorption of the estrogen. While systemic estrogen is metabolized by liver enzymes, the clinical impact of liver enzyme inducers (like carbamazepine or rifampin) on Estring’s low local dose is generally considered negligible.

Estring Vaginal Ring is contraindicated during pregnancy and must be removed immediately if pregnancy is suspected. Estrogens pass into breast milk and may reduce the quantity and quality of milk produced; therefore, use during breastfeeding is generally not recommended. It is recommended to speak to your doctor immediately if you are planning to get pregnant.

In women with an intact uterus, the addition of a progestogen should be discussed with a healthcare provider to minimize the long-term risk of endometrial hyperplasia or cancer from estrogen exposure.

Store Estring Vaginal Ring at controlled room temperature (15°C to 25°C or 59°F to 77°F) in its original, sealed protective pouch until you are ready to insert it. Avoid storing the medication in areas exposed to excessive heat or direct sunlight. Keep out of reach of children and pets.
