Arthritis or Diabetes?

A collaborative group of researchers including the American Diabetes Association and the Juvenile Diabetes Research Foundation has been testing the medication abatacept (CTLA4 immunoglobin fusion protein) as a possible treatment for type 1 diabetes. Abatacept, better known by its brand name Orencia, is FDA approved to treat autoimmune diseases such as rheumatoid arthritis and multiple sclerosis. 

Type 1 diabetes is an autoimmune disease in which T-cells in the body’s immune system mistakenly attack the insulin producing beta cells in the pancreas. With the pancreas producing little or no insulin, type 1 diabetics must rely on insulin injections to regulate their blood sugar levels. Those type 1 diabetes who continue to produce some insulin have an easier time keeping their blood sugar in the normal range, and have less risk of diabetes complications.

After URL Pharma won the exclusive rights to market the gout medication colchicine, over twenty other American pharmaceutical companies were ordered to stop manufacturing it. The price soared from nine cents a pill to almost $5, and Canadian pharmacies rushed to persuade American gout sufferers to buy colchicine from Canada.

Concerned that older drugs that have been in use since before the inception of a stringent approval process in the 1960′s may not meet current standards for safety, effectiveness, quality and labeling, the Food and Drug Administration (FDA) launched the Unapproved Drug Initiative in 2006.

Under this initiative, the consumer protection agency ordered pharmaceutical companies manufacturing unapproved drugs to either undergo rigorous and expensive clinical trials to obtain approval for the drugs, or to remove them from the market. In return, those companies winning approval would be given exclusive market rights for a time period, to allow them to recoup their investment.

When the American pharmaceutical company URL Pharma acquired the exclusive rights to manufacture a common gout drug that numerous other companies had been making for years, the price soared from 10¢ a pill to $5 a pill. What’s going on here?

Prescription colchicine is a pain reliever and anti-inflammatory derived from a crocus plant that has been in use since the 1800s. It’s primarily prescribed to treat gout, a painful form of arthritis, but is also an effective treatment for familial Mediterranean fever.

Over twenty different US pharmaceutical companies were manufacturing generic colchicine up until 2010. That’s when the FDA ordered them to stop manufacturing the gout drug within 45 days, and to stop shipping it within 90 days.
