It is available in tablet form for oral use and as an injection for use in acute or emergency settings, typically administered once daily or up to twice daily.

The mechanism of action for bumetanide is potent. It occurs in the thick ascending limb of the loop of Henle in the kidney. Bumetanide works by inhibiting the Sodium-Potassium-2-Chloride (NKCC2) co-transporter system on the luminal side of the tubule. By blocking the reabsorption of these ions, bumetanide significantly increases the excretion of sodium, chloride, and water, leading to a profound diuretic effect. This rapid and substantial reduction in fluid volume helps reduce edema, pulmonary congestion (pulmonary edema), and the overall workload on the heart, making it critical for managing acute heart failure exacerbations.
