Repatha Information
Repatha (Evolocumab) is a prescription medication used primarily to lower cholesterol levels. It is used to reduce LDL cholesterol (bad cholesterol) levels in patients with lipid disorders, including primary hypercholesterolemia, mixed dyslipidemia, and genetic lipid disorders like familial hypercholesterolemia (both heterozygous and homozygous types). LDL cholesterol can accumulate along the walls of your arteries, forming plaque. Over time, this buildup can narrow the arteries, restricting blood flow to essential organs like the heart and brain. When blood flow becomes severely reduced or blocked, it can lead to severe conditions such as a heart attack, stroke, or other related health problems. This medication is usually used as an adjunct to dietary modifications and lifestyle changes, and sometimes with other lipid-lowering medicines.
Evolocumab is a versatile medication because it not only helps lower blood cholesterol but also prevents complications from lipid disorders involving high bad cholesterol. Evolocumab can also be used to reduce the risk of major adverse cardiovascular events such as cardiovascular death, myocardial infarction, stroke, and unstable angina, all associated with high blood cholesterol and other lipids.
Evolocumab is a human monoclonal antibody, specifically a PCSK9 inhibitor. Its mechanism of action differs from typical lipid-lowering medications such as statins. It basically works by targeting and inhibiting a specific protein called PCSK9 (ProProtein Convertase Subtilisin/Kexin type 9). Typically, the PCSK9 protein binds to LDL receptors on the surface of liver cells, which marks the receptors for degradation. Fewer receptors mean the liver can't clear as much LDL cholesterol (bad cholesterol) from the blood. By blocking PCSK9, Repatha helps to prevent this degradation. This means more LDL receptors remain on the liver's surface, allowing the liver to remove significantly more LDL cholesterol from the blood and, ultimately, lowering the circulating cholesterol levels.
While reducing LDL cholesterol levels in the blood is its primary benefit, it has also been found to cause about 8% increase in good cholesterol levels, further enhancing its effect in preventing adverse cardiovascular events.
