Aggrenox (Dipyridamole/ASA) Information

Aggrenox contains two active ingredients, dipyridamole and aspirin. It is used to lower the risk of stroke in people who have already had a transient ischemic attack (TIA) or an ischemic stroke. These ingredients help prevent blood clots from forming in the arteries that supply the brain, reducing the chance of another stroke. Aggrenox is taken by mouth as an extended-release capsule, usually twice a day, to maintain steady protection throughout the day.

The mechanism of action of Aggrenox relates to the complementary antiplatelet effects of its two components. Dipyridamole helps prevent platelets from becoming activated by increasing adenosine levels. Aspirin prevents platelets from sticking together by blocking the COX-1 enzyme, which reduces the production of thromboxane A2, a substance that promotes clot formation. The combination offers stronger protection against clot-related strokes than either ingredient alone. The extended-release form of dipyridamole provides smooth antiplatelet activity and reduces peaks that might cause side effects.

Aggrenox Side Effects

Aggrenox is generally well-tolerated, but some common side effects may occur. Common side effects include headache, dizziness, abdominal discomfort, indigestion, nausea, diarrhea, and flushing. Some people may notice easy bruising or minor nosebleeds due to its antiplatelet effects. Mild fatigue or muscle aches can also occur, particularly when starting treatment.

More serious side effects are less common but require immediate medical attention. Stop the medication and seek urgent care if you experience:
• Chest pain, irregular heartbeat, or shortness of breath
• Severe headaches, sudden weakness, speech changes, or confusion
• Severe stomach pain or persistent vomiting

• Signs of bleeding: vomiting blood, black or tarry stools, coughing up blood, or unusual bruising
• Tinnitus (ringing in the ears), which may indicate aspirin toxicity

Aggrenox Precautions

Do not use Aggrenox if you are allergic to dipyridamole, aspirin, or other NSAIDs, or if you have had aspirin-induced asthma or severe reactions to salicylates. People with bleeding disorders, stomach ulcers, recent gastrointestinal bleeding, uncontrolled high blood pressure, liver or kidney disease, or certain heart conditions may need special evaluation before starting treatment. People with a history of migraines may notice worsening headaches when beginning therapy.

Tell your doctor about all medications you are taking, including prescription medicines, over-the-counter drugs, vitamins, and herbal supplements. Aggrenox can interact with blood thinners (such as warfarin, heparin, or DOACs), other antiplatelet medications, NSAIDs, SSRIs/SNRIs, gout medications, and blood pressure-lowering drugs. Drinking alcohol while taking Aggrenox may raise the risk of stomach bleeding, so intake should be limited or avoided. Inform your doctor if you are pregnant or planning to become pregnant. Aspirin-containing products should generally be avoided during pregnancy, especially in the third trimester, due to potential complications. If you are breastfeeding, discuss with your doctor, as aspirin can pass into breast milk and may affect the infant. Do not give this medication to children or teenagers with viral infections such as flu or chickenpox due to the risk of Reye’s Syndrome. Take Aggrenox exactly as prescribed, usually one capsule twice daily. Swallow the capsules whole without chewing or opening them, as this may interfere with the extended-release mechanism. Taking the medication with food may help reduce stomach discomfort. Store the medication at room temperature, away from heat and moisture, and keep it out of the reach of children.
