Benadryl Information

Benadryl (diphenhydramine) is a common over-the-counter (OTC) medication used to treat a variety of conditions, primarily related to allergic reactions. As a first-generation antihistamine, its active ingredient, diphenhydramine hydrochloride, works by blocking the effects of histamine, a natural substance in the body that triggers allergic symptoms. Specifically, it acts as an inverse agonist at the H1 histamine receptor, reversing the effects of histamine on capillaries and smooth muscles, thereby reducing symptoms like sneezing, a runny nose, and itching.

Beyond its role as an antihistamine, Benadryl also has other pharmacological properties. It possesses strong anticholinergic (drying) effects, which can help to alleviate symptoms such as a runny nose and watery eyes. These anticholinergic properties are also responsible for some of its side effects, like dry mouth and blurred vision. Additionally, because diphenhydramine readily crosses the blood-brain barrier, it can cause central nervous system (CNS) depression, leading to drowsiness. This sedative effect is why Benadryl is also used as a short-term nighttime sleep aid.

Diphenhydramine is used to treat a range of conditions, including hay fever, other upper respiratory allergies, and the common cold. It is also effective in providing relief from itching and pain associated with insect bites and stings, hives, and other skin irritations. In some cases, injectable forms of diphenhydramine are used in clinical settings for more severe allergic reactions or to manage symptoms of motion sickness and certain movement disorders, such as drug-induced extrapyramidal symptoms or Parkinsonism.

Benadryl Side Effects

As with any medication, Benadryl can cause a range of side effects, some of which are more common and others that are serious but rare. The most frequently reported side effects are due to its sedative and anticholinergic properties. These may include drowsiness, dizziness, and difficulty with coordination. Patients may also experience a dry mouth, nose, or throat, dry eyes, blurred vision, constipation, and stomach upset. It is important to note that while Benadryl is known to cause drowsiness, it can have the opposite effect in some individuals, particularly children and some older adults, leading to excitability, nervousness, or restlessness.

Although less common, more severe side effects can occur. You should stop using the medication and seek immediate medical attention if you experience any of the following serious side effects:

• Central Nervous System Effects: In some cases, diphenhydramine can cause paradoxical CNS stimulation, especially at high doses or in children. This can manifest as agitation, anxiety, confusion, or hallucinations. An overdose can lead to seizures, coma, and even death.

• Cardiovascular Effects: Rare but serious cardiovascular side effects have been reported, including an increased heart rate (tachycardia) and heart palpitations. In some individuals, it may prolong the QT interval, which can lead to serious cardiac arrhythmias.

• Urinary Retention: Due to its anticholinergic action, Benadryl can cause or worsen urinary retention, making it difficult to urinate, especially in men with an enlarged prostate.

• Severe Allergic Reactions: Although Benadryl is used to treat allergic reactions, a severe allergic reaction to the medication itself is possible, albeit rare. Symptoms can include a widespread rash, hives, severe itching, or swelling of the face, lips, and tongue, along with difficulty breathing.

Benadryl Precautions

Before taking Benadryl, it is essential to consult a healthcare provider and disclose your complete medical history. It should not be used in newborn or premature infants. It should also be avoided if you are allergic to diphenhydramine or other similar antihistamines. Individuals with certain pre-existing conditions should use this medication with caution, including those with narrow-angle glaucoma, a history of asthma, chronic bronchitis, or other respiratory problems, as the anticholinergic effects can thicken mucus and make breathing more difficult.

Due to the risk of sedation and impaired coordination, you should avoid driving, operating heavy machinery, or engaging in other activities that require mental alertness until you know how the medication affects you. Alcohol and other central nervous system depressants (e.g., sedatives, tranquillizers) can significantly increase the sedative effects of Benadryl and should be avoided. Benadryl should not be taken if you are currently using a monoamine oxidase inhibitor (MAOI) or have used one within the last two weeks, as this can lead to a dangerous drug interaction.

If you are pregnant or planning to become pregnant, you should discuss the use of Benadryl with your doctor. While animal studies have not shown harm to the fetus, there are no adequate and well-controlled studies in pregnant women. Additionally, diphenhydramine can pass into breast milk and may cause undesirable effects in a nursing infant. Its use while breastfeeding should generally be avoided as it may also decrease milk supply.

The use of Benadryl as a sleep aid is only intended for short-term, occasional sleeplessness. If insomnia persists for more than two weeks, it could be a symptom of a serious underlying medical condition, and you should seek medical advice. Additionally, due to its side effect profile, Benadryl is listed in the Beers Criteria as a potentially inappropriate medication for older adults, as it can increase the risk of dizziness, sedation, and falls.
