Cortef (Hydrocortisone) Information

Cortef, which contains the active ingredient hydrocortisone, is an oral corticosteroid medication. It is primarily used as hormone replacement therapy for individuals whose adrenal glands do not produce enough natural cortisol, such as those with Addison's disease or primary adrenocortical insufficiency. It is also used to treat various inflammatory, allergic, and autoimmune conditions, including severe asthma, arthritis, and certain skin disorders. The medication is taken by mouth as a tablet, usually 2 to 3 times daily, to mimic the body's natural daily rhythm of cortisol production. The largest dose is typically taken in the morning. Your doctor will determine the lowest effective dose for your specific condition. It is important to take Cortef with food or milk to help prevent stomach irritation.

The mechanism of action involves acting as a substitute for the natural glucocorticoid hormones produced by the adrenal cortex. It works by suppressing the immune system and reducing the production of inflammatory chemicals. In replacement therapy, it restores the body's hormonal balance, which is essential for regulating metabolism, blood pressure, and the body's response to stress.

Cortef Side Effects

Cortef is generally well-tolerated; side effects are more common with long-term use or higher doses of the medication. Commonly reported effects include increased appetite, weight gain, insomnia, and “puffy” face (moon face). More serious side effects can occur and require immediate medical attention.

Stop taking the medication and seek help if you experience:
• High Blood Sugar: Increased thirst and frequent urination.
• Psychological Changes: Severe depression, anxiety, or unusual changes in personality or behaviour.
• Severe Allergic Reactions: Swelling of the face, lips, or throat; hives; or significant difficulty breathing.
• Signs of Infection: Fever, chills, or a persistent sore throat, as Cortef can mask symptoms of a serious infection.
• Vision Changes: Blurred vision, eye pain, or seeing halos, which may indicate glaucoma or cataracts.

Cortef Precautions

Do not use Cortef if you have a known allergy to hydrocortisone or any of the inactive ingredients in the medication. Do not use this medication if you have a systemic fungal infection. Before starting treatment, inform your doctor of your complete medical history, especially if you have high blood pressure, osteoporosis, stomach ulcers, or diabetes.

Tell your doctor about all medications you are taking, including prescription drugs, over-the-counter medicines, vitamins, and herbal supplements. Cortef has significant interactions with many drugs, especially blood thinners (like warfarin), anti-seizure medications, NSAIDS, and certain antibiotics. You should also avoid getting live vaccines while taking this medication, as your immune response may be weakened.

Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Cortef should only be used during pregnancy if the potential benefit outweighs the risk to the fetus. Corticosteroids can pass into breast milk and may affect a nursing infant's growth or natural hormone production. Consult your doctor before breastfeeding.

Take Cortef exactly as your doctor tells you. Swallow the tablets whole with a glass of water. Take this medicine with food to avoid stomach upset. If you are using Cortef for adrenal insufficiency, your body may require higher doses during times of physical stress, such as surgery, serious injury, or severe illness. Discuss a "stress dose" plan with your doctor and consider wearing a medical alert bracelet. Do not stop taking Cortef suddenly if you have been on it for more than a few weeks; your doctor will provide a taper schedule to gradually reduce the dose and allow your adrenal glands to resume normal function. Store the tablets at room temperature, away from moisture and light, and keep them safely out of reach of children.
