Florinef (Fludrocortisone) Information

Florinef, which contains the active ingredient fludrocortisone, is a potent synthetic corticosteroid primarily a mineralocorticoid, with minimal glucocorticoid activity. It is most commonly prescribed for Addison's disease and the salt-losing form of congenital adrenal hyperplasia (adrenogenital syndrome). Additionally, it is used as a first-line treatment for orthostatic hypotension (a sudden drop in blood pressure when standing) and postural orthostatic tachycardia syndrome (POTS) because it helps increase blood volume.

The mechanism of action involves mimicking the natural hormone aldosterone produced by the adrenal glands. It binds to mineralocorticoid receptors in the kidneys, prompting the body to reabsorb sodium and water while increasing potassium excretion. This process expands blood volume and raises blood pressure, helping to maintain electrolyte balance and prevent dehydration.

Florinef Side Effects

Florinef is generally well-tolerated, but common side effects may occur as your body adjusts to the medication. Commonly reported effects include headaches, dizziness, mood changes such as anxiety, irritability, or trouble sleeping. Some patients may notice a rounding of the face (moon face).

More serious side effects can occur and require immediate medical attention. Seek help if you experience any of the following side effects:

Florinef Precautions

Do not use Florinef if you have a known allergy to fludrocortisone or any of the inactive ingredients in the medication. Before starting treatment, inform your doctor of your complete medical history, especially if you have heart disease, high blood pressure, or kidney problems, as the drug's fluid-retaining effects can worsen these conditions. You should also disclose a history of glaucoma, osteoporosis, stomach ulcers, or mental health disorders like depression or psychosis. Tell your doctor if you have any current or past infections, particularly tuberculosis (TB) or herpes simplex of the eye, as steroids can mask or reactivate latent infections. Patients with diabetes must be monitored closely, as Florinef can raise blood sugar levels.

Florinef interacts with several medications, most notably diuretics (water pills) and amphotericin B, which can dangerously worsen potassium loss. Infants born to mothers taking Florinef should be monitored for signs of adrenal insufficiency. It is not known whether fludrocortisone passes into breast milk in significant amounts. However, corticosteroids are known to pass into breast milk. While the risk to a nursing infant is generally considered low at standard doses, caution is recommended, and the infant should be observed for signs of suppressed growth or adrenal function. Use Florinef exactly as your doctor tells you. Florinef tablets are typically taken once daily, preferably in the morning to mimic the body's natural hormone rhythm. Your doctor may recommend a low-sodium (low-salt) diet and potassium supplements to help manage the drug's effects on your electrolyte balance. During times of physical stress, such as surgery, severe illness, or injury, your doctor may need to temporarily increase your dose to prevent an adrenal crisis. Because this medication is essential for hormone replacement, you should never stop taking it without consulting your doctor. Store Florinef at room temperature (20°C to 25°C) in a dry place away from direct light and moisture. Ensure the container is kept tightly closed when not in use. Keep safely out of the reach of children.
