Fragmin Injection (Dalteparin) Information
Fragmin injection, which contains the active ingredient dalteparin, is a low–molecular–weight heparin (LMWH). It is used to prevent and treat harmful blood clots. It is commonly prescribed for the prevention of deep vein thrombosis (DVT) in patients undergoing surgery or those who are bedridden due to acute illness. Fragmin is also used in the treatment of DVT and pulmonary embolism (PE), particularly in patients with cancer, and may be used in patients with certain types of unstable heart conditions such as unstable angina or non–Q-wave myocardial infarction.
Fragmin is administered by subcutaneous injection, usually once daily, although the dose varies depending on the condition being treated. Its predictable absorption and long duration of effect allow for consistent anticoagulation without the frequent blood monitoring required with traditional heparin.
The mechanism of action of dalteparin involves enhancing antithrombin's activity, a protein that helps block clotting factors, especially Factor Xa. By reducing Factor Xa activity, dalteparin slows clot formation and lowers the risk of thrombosis in both veins and arteries. Its more focused effect contributes to lower bleeding risk and more stable anticoagulation than older forms of heparin. Because it begins working quickly and maintains steady levels, it is widely used both in hospital and outpatient settings.
Fragmin Injection Side Effects
Fragmin injection is generally well tolerated, but common side effects may occur. Common side effects include mild pain, redness, bruising, or irritation at the injection site. Some people may experience mild bleeding from the nose or gums, nausea, or fatigue. Small bumps or firmness under the skin at the injection site can also occur.
More serious side effects can occur, leading to significant monitoring protocols. If you experience severe symptoms, seek emergency care, but do not stop the injection unless instructed by a doctor. Symptoms requiring immediate medical attention include:
• Chest pain, difficulty breathing, or sudden swelling in the legs (possible blood clot)
• Signs of severe allergic reactions, including swelling of the face or throat, difficulty breathing, rash
• Signs of severe bleeding, including black or tarry stools, blood in urine, coughing or vomiting blood, or unusual bruising
• Signs of spinal or epidural bleeding, especially after procedures, including back pain, numbness, tingling, or loss of bladder/bowel control
• Sudden severe headache, dizziness, or weakness (possible internal bleeding)
• Symptoms of heparin-induced thrombocytopenia (HIT), including new or worsening clots, or suddenly low platelet counts
Fragmin Injection Precautions
Do not use Fragmin if you are allergic to dalteparin, heparin, or pork-derived products, or if you have active major bleeding. You must not take this medicine if you have a history of heparin-induced thrombocytopenia (HIT). Before starting treatment, inform your doctor of any history of bleeding disorders, recent surgeries, strokes, spinal procedures, high blood pressure, stomach ulcers, kidney disease, liver disease, or low platelet counts. You may require blood tests to monitor for HIT or assess kidney function during treatment.
Tell your doctor about all medications you are taking, including prescription drugs, over-the-counter medications, supplements, and herbal products. Fragmin can interact with blood thinners (warfarin, DOACs), antiplatelet agents (aspirin, clopidogrel), NSAIDs, and certain supplements such as fish oil, ginkgo, or garlic, all of which may increase the risk of bleeding. Alcohol may also raise the likelihood of bleeding and should be limited.
Inform your doctor if you are pregnant or planning to become pregnant. Fragmin is often preferred in pregnancy for clot prevention and treatment, but dosing must be carefully managed. Pregnant patients typically receive the preservative-free single-dose syringes. Small amounts of dalteparin may pass into breast milk. If you are breastfeeding, discuss the risks and benefits of receiving this medicine with your doctor.
Use Fragmin injection exactly as prescribed. It is typically given subcutaneously once daily. Your doctor or nurse will show you how to inject it safely. Do not remove the air bubble from the pre-filled syringe before injection. The bubble is designed to push all the medication in and prevent leakage. Rotate injection sites and avoid rubbing the area after administration to reduce bruising. Do not stop using Fragmin suddenly unless your doctor instructs you to do so. Stopping this medicine suddenly may increase the risk of blood clots.
Store Fragmin at room temperature, away from heat and moisture, and keep it out of reach of children.
