Minims Pilocarpine (Pilocarpine nitrate) Information
Minims Pilocarpine, which contains the active ingredient pilocarpine nitrate, is a prescription ophthalmic solution primarily used to lower intraocular pressure in patients with glaucoma or ocular hypertension. It is a cholinergic agonist also used to constrict the pupil (miosis) after it has been dilated during an eye exam or to treat certain types of acute angle-closure glaucoma. Do not use Minims Pilocarpine if you have uveitis or acute iritis.
The mechanism of action involves mimicking the action of acetylcholine, a natural chemical in the body, to stimulate the "muscarinic" receptors in the eye. This causes the iris sphincter muscle to contract, resulting in pupil constriction. Simultaneously, it causes the ciliary muscle to contract, which opens the eye's drainage channels (the trabecular meshwork). This allows the fluid inside the eye (aqueous humor) to flow out more easily, thereby reducing intraocular pressure.
Minims Pilocarpine Side Effects
Minims Pilocarpine is generally well-tolerated, but common side effects may occur as your body adjusts to the medication. Commonly reported effects include temporary blurred vision (especially at night), stinging or burning upon application, and a browache or headache caused by the contraction of the eye muscles. You may also notice reduced vision in low light because the pupil is smaller.
More serious side effects can occur and require immediate medical attention. Stop taking the medication and seek help if you experience:
• Retinal Detachment: Signs include a sudden increase in floaters, flashes of light, or a curtain-like obscuration of vision (a rare but serious risk in near-sighted patients).
• Severe Allergic Reactions: Swelling of the face, lips, or throat; hives; or significant difficulty breathing.
• Severe Eye Pain: Intense pain, redness, or a sudden change in vision clarity.
• Systemic Cholinergic Effects: Rare symptoms like increased sweating, salivation, nausea, diarrhea, or shortness of breath may occur if the drug is absorbed into the bloodstream.
Minims Pilocarpine Precautions
Do not use Minims Pilocarpine if you have a known allergy to pilocarpine nitrate or any of the inactive ingredients in the medication. Before starting treatment, inform your doctor of your complete medical history, especially if you have asthma, heart disease, stomach ulcers, Parkinson's disease, or retinal disease.
Tell your doctor about all medications you are taking, including prescription drugs, over-the-counter medicines, vitamins, and herbal supplements. Minims Pilocarpine has significant interactions with many drugs, especially with anticholinergic drugs, glaucoma medications, or medications used during surgery.
Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. It is unknown if this medicine is safe for use during pregnancy. Use Minims Pilocarpine in pregnancy only if the benefits outweigh the risks. It is unknown whether pilocarpine nitrate is excreted into human milk. Apply pressure to the tear duct after application to minimize absorption.
Use Minims Pilocarpine exactly as your doctor tells you. Wash your hands and instill one drop into the affected eye(s). To reduce absorption into the body, press on the inner corner of the eye (near the nose) for at least one minute after the drop is placed. Use caution when driving at night or performing tasks in poor lighting, as your vision may be significantly reduced. Remove contact lenses before use and wait at least 15 minutes before reinserting. Store in the fridge (2–8°C) and protect from light. Discard the unit immediately after a single use.
