Pepto Bismol Information
Pepto Bismol (Bismuth) is an antidiarrheal and mucosal protective agent used to treat minor, temporary digestive issues. It is used to reduce the frequency and fluidity of loose stools in acute, non-specific diarrhea. It is used both to prevent and treat diarrhea caused by consuming contaminated food or water during travel. It is also used to relieve the burning sensation in the chest or upper abdomen and eases general stomach upset. Furthermore, it is used in combination with specific antibiotics and acid suppressants to help eliminate Helicobacter pylori, a bacterium responsible for most peptic ulcers. It may also be used to control the feeling of sickness or the urge to vomit. Inside the stomach, gastric acid splits bismuth subsalicylate into two distinct active components that target gastrointestinal symptoms in separate ways.
Salicylic acid is the anti-inflammatory and antisecretory component, which, after absorption into the lining of the digestive tract, inhibits the production of prostaglandins, which are the chemicals responsible for intestinal inflammation. Suppressing these chemicals prevents the intestines from pumping excess water and electrolytes into the bowel, effectively reducing the volume of fluid in diarrhea. Bismuth, with its antimicrobial & protective effect, remains largely unabsorbed and stays within the digestive tract. It exerts direct bactericidal effects in the gut by disrupting bacterial cell walls and inhibiting protein synthesis. Furthermore, bismuth binds to and neutralizes bacterial toxins while forming a physical coating over irritated mucosal linings and ulcers, protecting them from gastric acid and allowing the tissue to heal.
Pepto Bismol Side Effects
While generally safe for short-term use, Pepto Bismol carries some risks of temporary or mild side effects such as constipation, dark tongue, grey or black stools, and headaches.
Check with your doctor as soon as possible if you experience serious side effects such as anxiety, muscle spasms, muscle weakness, or severe constipation.
Seek immediate medical attention if you notice the following side effects:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, swelling of the face, mouth, tongue, lips, or throat;
• Symptoms of taking too much of this medication (salicylate toxicity) include seizures, extreme drowsiness, fast breathing, ringing in the ears, confusion, and hearing loss.

Pepto Bismol Precautions
Do not use this medication if you are allergic to bismuth subsalicylate, aspirin, or other salicylates, or any inactive ingredients in Pepto Bismol or any other medication. Before using this medication, ensure you inform your doctor about any medical conditions you previously or currently have, including the following:
• A history of Reye Syndrome;
• Arthritis;
• Bleeding problems;
• Dehydration;
• Diabetes;
• Diarrhea with fever and mucus in stool;
• Gout;
• Kidney disease;
• Liver disease;
• Peptic ulcers;
• Viral infections like chickenpox or influenza, especially in children and teenagers
Do not give this medication to children or teenagers under 12 years of age. It is strictly contraindicated for minors recovering from viral infections like chickenpox or influenza, as salicylates are heavily linked to Reye’s syndrome—a rare but life-threatening condition that causes swelling in the liver and brain. Concomitant use of this medication with blood thinners like warfarin, heparin, direct oral anticoagulants (DOACs), or antiplatelet therapies (like clopidogrel) significantly increases the risk of gastrointestinal bleeding. Likewise, taking it alongside aspirin, ibuprofen, naproxen, or other NSAIDs compounds the total salicylate burden, significantly escalating the risk of ulcers, gastrointestinal bleeding, and systemic salicylate toxicity (which can manifest early as tinnitus). Therefore, it is imperative to inform your doctor about any other medications you are currently taking, including herbal medications and supplements, to determine if it is safe to use along with Pepto Bismol.
Bismuth subsalicylate is not recommended during pregnancy, particularly after 20 to 30 weeks. It crosses the placental barrier and carries a risk of causing intrauterine growth restriction, premature closure of the fetal ductus arteriosus, and maternal or fetal bleeding complications. It is generally avoided during pregnancy and while breastfeeding, as salicylates can pass through breastmilk and affect the breastfed infant. Store this medication at room temperature, protect it from light and moisture, and keep it out of the reach of children.
