Prolensa (Bromfenac sodium) Information

Prolensa, which contains the active ingredient bromfenac sodium, is a prescription non-steroidal anti-inflammatory drug (NSAID) specifically used to treat inflammation and reduce eye pain in patients who have undergone cataract surgery. It is a once-daily treatment designed for comfort, with a pH similar to that of human tears to minimize stinging. Prolensa treatment begins one day before surgery, continues on the day of surgery, and lasts for 14 days post-op. It is important to note that Prolensa is not intended for long-term use, as prolonged application can increase the risk of serious corneal issues.

The mechanism of action involves inhibiting cyclooxygenase enzymes (COX-1 and COX-2). These enzymes produce prostaglandins, natural substances in the body that cause pain, swelling, and inflammation in response to tissue trauma, such as surgery. By blocking these enzymes, Prolensa effectively stops the inflammatory cascade before it begins.

Prolensa Side Effects

Prolensa is generally well-tolerated, but common side effects may occur as your body adjusts to the medication. Commonly reported effects include blurred vision, eye pain, a foreign body sensation (feeling like something is in the eye), and increased sensitivity to light. You may also experience temporary stinging or burning immediately after application, increased tearing, or a mild headache.

More serious side effects can occur and require immediate medical attention. Stop taking the medication and seek help if you experience:
• Corneal Damage (Keratitis): Signs include severe redness, significant vision loss, or halos around lights, which may indicate thinning or ulceration of the cornea.
• Increased Bleeding: There is a potential for increased bleeding of ocular tissues during or after surgery.
• Severe Allergic Reactions: Symptoms such as hives, swelling of the face or throat, or difficulty breathing.
• Slow or Delayed Healing: Topical NSAIDs can slow down the eye's natural healing process, especially when used with steroid eye drops.

Prolensa Precautions

Do not use Prolensa if you have a known allergy to bromfenac sodium or any of the inactive ingredients in the medication. Before starting treatment, inform your doctor of your complete medical history, especially if you have asthma or a history of sulfite sensitivity, as the solution contains sodium sulfite, which can trigger severe allergic reactions in susceptible people. You should also disclose a history of bleeding problems, diabetes, rheumatoid arthritis, or dry eye syndrome, as these conditions can increase the risk of corneal complications. Also, mention any previous allergic reactions to aspirin or other NSAIDs due to the risk of cross-sensitivity.

Tell your doctor about all medications you are taking, including prescription drugs, over-the-counter medicines, vitamins, and herbal supplements. Prolensa is often used in conjunction with other eye medications, such as topical corticosteroids. However, this combination can significantly increase the risk of delayed healing and corneal thinning. It may also interact with blood thinners (such as warfarin), potentially increasing bleeding time in the eye. If you are using other eye drops, you must wait at least 5 minutes between applications.

Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Prolensa should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is strictly recommended to avoid use during late pregnancy (third trimester), as NSAIDs can cause the premature closure of the ductus arteriosus in the fetus. It is not known whether bromfenac sodium passes into breast milk. Caution is advised for breastfeeding mothers. While systemic exposure is low after using eye drops, you should consult your doctor before use.

Use Prolensa exactly as your doctor tells you. To apply Prolensa, wash your hands thoroughly, then instill 1 drop into the affected eye once daily as directed. Do not wear contact lenses during treatment unless your doctor specifically instructs you to do so. If you do wear soft contacts, remove them before instilling the drops and wait at least 10 to 15 minutes before reinserting them, as the lenses can absorb the preservative. Most formulations of Prolensa contain benzalkonium chloride, a preservative that can discolor soft contact lenses, which is why the 15-minute wait time is necessary. To avoid contamination, do not touch the dropper tip to your eye, fingertips, or any other surface. After applying the drop, gently press on the inner corner of the eye (tear duct) for 1 to 2 minutes to keep the medicine in the eye and reduce systemic absorption. Store the bottle at room temperature (15°C to 25°C) and keep it tightly closed when not in use. Always keep the medication safely out of the reach of children.
