Rexulti (Brexpiprazole) Information
Rexulti, which contains the active ingredient brexpiprazole, is an atypical antipsychotic used to treat schizophrenia and as an add-on treatment for major depressive disorder (MDD) when antidepressants alone are not enough. It helps stabilize mood, reduce hallucinations or delusions, improve clarity of thought, and lessen symptoms such as social withdrawal and emotional flatness. In depression, it is used alongside an antidepressant to help improve overall mood, energy, and daily functioning. Rexulti is taken by mouth once daily, with or without food.
The mechanism of action for brexpiprazole involves adjusting the balance of key neurotransmitters in the brain. It partially stimulates specific dopamine and serotonin receptors while blocking others. This balanced activity helps calm overstimulated pathways linked to psychosis and mood instability while supporting pathways that regulate motivation, mood, and decision-making. Because it does not entirely block dopamine, it tends to cause fewer movement-related side effects than older antipsychotics, though careful monitoring is still essential.
Rexulti Side Effects
Rexulti is generally well-tolerated, but some common side effects may occur. Commonly reported effects include sleepiness or fatigue, dizziness, weight gain, increased appetite, restlessness, and headache. Some individuals may also notice anxiety, difficulty sleeping, or mild digestive symptoms such as nausea or stomach discomfort.
More serious side effects can occur, leading to significant monitoring protocols. If you experience any of the following symptoms, stop taking the medication and seek immediate medical attention:
• High blood sugar: extreme thirst, frequent urination, confusion, fruity breath odor
• Low blood pressure: feeling faint, dizzy, or lightheaded
• Neuroleptic Malignant Syndrome (NMS): high fever, muscle stiffness, confusion, sweating, fast heartbeat
• Seizures
• Severe allergic reactions: swelling of the face or throat, rash, difficulty breathing
• Signs of severe restlessness (akathisia): inability to sit still, pacing, intense inner tension
• Sudden mood or behavior changes, including worsening depression or suicidal thoughts
• Tardive Dyskinesia (TD): involuntary movements of the face, tongue, or limbs
Rexulti Precautions
Do not take rexulti if you are allergic to brexpiprazole or any of its ingredients. Before starting treatment, inform your doctor about your complete medical history. People with diabetes, high cholesterol, heart disease, low blood pressure, seizures, a history of stroke, Parkinson’s disease, or trouble swallowing may require careful monitoring. You may need routine check-ups and lab tests because this medication can affect blood sugar, lipids, and weight.
Tell your doctor about all medicines you take, including antidepressants, mood stabilizers, antipsychotics, sedatives, opioids, antihistamines, and medications that affect blood pressure. Certain drugs can increase sedation, raise the risk of movement problems, or alter how rexulti is processed in your body, requiring dose adjustments. Avoid alcohol because it increases sedation and impairs judgment.
If you are pregnant, planning to become pregnant, or breastfeeding, discuss this with your doctor. Antipsychotic use during the last trimester of pregnancy may affect the newborn, so your doctor will weigh the benefits against the risks. Brexpiprazole can pass into breast milk, so shared decision-making is essential when considering breastfeeding while on this medication.
Use this medicine exactly as prescribed by your doctor. Rexulti is available as tablets. Take the tablets once daily, at the same time each day. You can take it with or without food. Your doctor will start you on a low dose and increase it gradually based on your response and tolerability. Do not change your dose or stop taking rexulti without medical guidance, as sudden discontinuation may lead to symptom return or withdrawal effects. Because rexulti can cause drowsiness, caution is needed when performing tasks that require alertness until you know how the medication affects you. Store the tablets at room temperature, away from moisture, heat, and light, and keep them out of reach of children.
