Timoptic XE (Timolol Maleate Gellan Solution) is used to reduce intraocular pressure in people with open-angle glaucoma or ocular hypertension. Glaucoma is a progressive disease where excessive fluid buildup creates pressure that can damage the optic nerve, potentially leading to irreversible vision loss if not treated. Its active ingredient is timolol maleate, a non-selective beta-adrenergic receptor blocker.
This medication reduces aqueous humor production, the clear fluid that fills the anterior chamber of the eye, thereby lowering intraocular pressure. This action helps relieve symptoms of increased intraocular pressure and helps prevent further nerve damage and preserve vision. Interestingly, this medication is a unique extended-release formulation of timolol maleate that contains gellan gum, which appears to transition from a liquid to a gel upon contact with the eye's natural salts. This transition allows the drug to remain in contact with the eye longer, enabling once-daily dosing that may be more convenient than traditional drops.
Timoptic XE Side Effects
Most people experience only minor, localized effects that typically resolve shortly after the drop is administered. Some of these include blurred vision (temporary) after putting in the eye drop, double vision, sensation of something in your eyes, stinging of the eye, or other eye irritation (when medication is applied). Some people have also experienced different side effects like nausea, headache, vomiting, and mild abdominal pain. Contact your doctor if these mild side effects persist or worsen.
Stop using this medicine and contact your doctor immediately if you experience any serious side effects, including the following:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, swelling of the face, mouth, tongue, lips, or throat;
• Low blood pressure;
• Muscle pain;
• Shortness of breath;
• Slow or irregular heartbeat

Timoptic XE Precautions
Do not use this medication if you are allergic to timolol maleate or any other ingredient in this topical medication. Before using this medication, ensure you inform your doctor about any medical conditions you previously or currently have, including the following:
• Allergic conditions such as asthma or atopic dermatitis;
• Breathing problems;
• Heart disease, including heart rate abnormalities and heart failure;
• Poor blood circulation problems (such as Raynaud's syndrome);
• Thyroid problems
Consult your doctor before using this medication during pregnancy to determine its risks and benefits. However, it is not recommended for use in breastfeeding women as it can pass through the breastmilk and harm the infant. Inform your doctor that you are on this medication if you are going for surgery, develop an eye infection, or have an eye injury. Soft contact lenses can absorb the preservative in Timoptic XE (benzalkonium chloride) and may discolor them or irritate the eye. You usually need to wait at least 15 minutes after the drop before putting your lenses back in.
Using this medication along with certain heart medications (like diltiazem or verapamil) can lead to severe disturbances in heart rhythm or low blood pressure. Therefore, it is essential to inform your healthcare provider of all medications, including other topical eye medications and over-the-counter medications, that you are taking before using timolol maleate to assess potential drug interactions. This medication is not recommended for use in children, as its safety and efficacy have not been established in this population.
Apply this eye drop medication into your eye as directed by your doctor or as stated in the medication pack. The usual dose for adults is one drop of this topical eye drop once daily, in one or both eyes. Your doctor will determine the appropriate dose for you depending on your condition. If you use other eye drops, wait at least ten minutes after using Timoptic XE before applying these topical medicines. Store this medication at room temperature and keep it away from children and pets.
