Vagifem is an estrogen therapy (vaginal Hormone Replacement Therapy) used to treat moderate to severe symptoms of menopausal vaginal atrophy, including vaginal dryness, burning, soreness, itching, and irritation. It works by delivering a localized dose of estradiol, a naturally occurring female sex hormone, directly to the vaginal mucosa. The small tablet is cleanly inserted via a disposable applicator, releasing estrogen right where it is needed to replace the natural hormones lost during menopause and rebuild the vaginal lining.
Side Effects
Commonly reported side effects include abdominal or stomach pain, back pain, bloating, nausea, vomiting, headache, dizziness, and a feeling of fatigue. You may also experience peripheral edema (swelling of the hands, arms, or feet), minor weight gain, breast pain, tenderness or enlargement, vaginal itching or irritation, vaginal discharge, and localized fungal or yeast infections.
Contact your doctor if you have any of the serious side effects, including:
• Cardiovascular Disorders: An elevated risk of developing serious blood clots, including deep vein thrombosis (DVT) or pulmonary embolism (PE), as well as a higher risk of suffering a stroke or heart attack, is associated with systemic (oral) estrogen, but clinical studies show no significant increase in these risks with localized Vagifem use.
• Malignant Neoplasms: Potential increased risk of developing breast cancer or ovarian cancer with continuous usage is a warning for systemic hormone therapy; data do not demonstrate an increased risk of these cancers with low-dose vaginal estradiol tablets.
• Probable Dementia: Possible higher risk of cognitive decline and dementia, particularly observed in postmenopausal women 65 years of age or older, applies to oral estrogen therapy, not localized vaginal therapies.
• Uterine Malignancies: An increased long-term risk of developing endometrial hyperplasia or cancer of the uterine lining due to unopposed estrogen exposure in women who still have a uterus applies to systemic estrogen. Clinical data show that the tiny amount of estrogen absorbed from Vagifem does not significantly stimulate the uterine lining.
Seek medical attention immediately if you experience abnormal or unexplained vaginal bleeding, discover new lumps or dimpling in your breast, or notice severe localized pain and swelling in the vagina. You must also get urgent emergency care if you experience sudden leg pain or warmth, severe chest pain, coughing up blood, sudden shortness of breath, a severe unexpected headache, or signs of a serious allergic reaction such as a widespread skin rash, hives, difficulty breathing, or swelling of the lips, throat, or tongue. Do not stop using this medication suddenly if you are on a long-term therapeutic schedule without consulting your doctor first, as your uncomfortable menopausal vaginal symptoms may quickly return.
Precautions:
Do not use Vagifem if you have a known hypersensitivity or serious allergic reaction to estradiol or any of the inactive ingredients in the tablet coating or base matrix, such as hypromellose or polyethylene glycol. Tell your doctor about your medical history, especially if you have:
o Active liver disease, liver problems, or a history of impaired liver function.
o Bleeding or blood-clotting disorders.
o Current, suspected, or historical breast cancer, uterine cancer, or other estrogen-dependent tumors.
o History of a recent heart attack, stroke, or transient ischemic attack (TIA).
o Known or suspected pregnancy.
o Previous history or current presence of active blood clots.
o Undiagnosed or unexplained abnormal vaginal bleeding.
Vagifem can interact with medications that induce liver enzymes, which may accelerate the clearance of estradiol and reduce its clinical efficacy; check with your doctor if you take anticonvulsants or rifampin. Additionally, because Vagifem bypasses the digestive system, it does not interact with grapefruit products, and they do not need to be avoided.
Vagifem is strictly contraindicated during pregnancy and must not be used under any circumstances; if you become pregnant or think you might be, stop using the inserts and tell your doctor immediately. This medication passes into breast milk and can reduce the absolute quality and volume of milk. It is not recommended for use while breastfeeding. It is recommended to speak to your doctor immediately if you are planning to get pregnant. (Note: Vagifem is intended for postmenopausal women, making pregnancy or breastfeeding concerns virtually non-existent in typical clinical practice).
When Vagifem is prescribed for a postmenopausal woman who still has an intact uterus, major medical organizations (including the North American Menopause Society) state that the concurrent use of a progestogen is generally NOT required, as Vagifem's ultra-low dose does not increase the risk of endometrial hyperplasia or uterine cancer.
Store Vagifem vaginal inserts at dry room temperature between 15°C and 25°C. Do not refrigerate or freeze the product. Keep each tablet securely sealed in its original blister pack until immediately before administration, to protect it from moisture and direct light. Always store the medication and its single-use applicators safely out of the reach of children and pets.
