Vasostrict (Vasopressin) Information

Vasostrict, which contains the active ingredient vasopressin (synthetic arginine vasopressin), is a potent vasoconstrictor and antidiuretic hormone. It is indicated to increase blood pressure in adults with vasodilatory shock (e.g., septic shock) who remain hypotensive despite fluid resuscitation and catecholamines (like norepinephrine). Vasopressin is administered intravenously (IV) as a continuous infusion, often in combination with catecholamine pressors, to help restore adequate perfusion to vital organs during life-threatening hypotensive states.

The mechanism of action for vasopressin involves the selective stimulation of two types of vasopressin receptors: V1 and V2. V1 Receptors are found on vascular smooth muscle cells, and their stimulation causes potent vasoconstriction (narrowing of blood vessels). This action is independent of the typical alpha-adrenergic pathways used by catecholamines. It is particularly effective in reducing nitric oxide-mediated vasodilation often seen in septic shock. V2 Receptors are found in the renal collecting ducts, and their stimulation increases water reabsorption by the kidney (the antidiuretic effect). In the context of shock, the primary therapeutic effect is potent, non-adrenergic-mediated vasoconstriction induced by V1 receptor stimulation, leading to increased systemic vascular resistance and blood pressure.

Vasostrict Side Effects

Vasostrict is used in critically ill patients, and its side effects are often related to its potent vasoconstrictive and antidiuretic actions. Common side effects frequently reported include decreased cardiac output, bradycardia (slow heart rate), abdominal cramping, and nausea.

However, more serious side effects can occur, often due to excessive or unbalanced vasoconstriction. If any of the following symptoms are experienced or noted during monitoring, the patient should receive immediate medical attention:
• Cardiac Arrest: Rapid changes in systemic vascular resistance can lead to myocardial ischemia, cardiac rhythm disturbances, and arrest.

• Ischemia (Peripheral and Organ): Due to its potent vasoconstrictor effect, vasopressin can cause severe, potentially irreversible ischemia (lack of blood flow) to the extremities (fingers/toes) and internal organs (e.g., mesenteric, coronary), leading to necrosis.
• Water Intoxication/Hyponatremia: Due to its antidiuretic effect (V2 stimulation), excessive water retention can lead to dangerously low sodium levels (hyponatremia), cerebral edema, and seizures.

Vasostrict Precautions

Vasostrict is for intravenous administration only and requires continuous monitoring in a critical care setting. It should be used with extreme caution in patients with known coronary artery disease or peripheral vascular disease due to the high risk of causing or worsening ischemia. It should be used only after adequate fluid resuscitation and is typically administered only when hypotension persists despite the use of catecholamine vasopressors. Fluid and electrolyte balance, particularly serum sodium levels, must be closely monitored due to the antidiuretic effect. If the drug is abruptly discontinued, the patient may experience a rapid drop in blood pressure (hypotension), so it must be tapered slowly. The extravasation (leakage out of the vein) of the infusion can cause severe local tissue necrosis.
