Xeomin Information
Xeomin (Botulinum Toxin Type A) is a versatile medication. It is currently used for several therapeutic indications in both children and adults, as well as for cosmetic purposes. In clinical settings, it is used mainly for chronic sialorrhea (excessive drooling) in adults and children (aged 2 and older). Xeomin is also indicated for upper limb spasticity in adults, and was recently approved in some regions for spasticity in children (ages 2–17), excluding cases related to cerebral palsy. It treats cervical dystonia, helping manage abnormal head position and neck pain. Xeomin also helps treat blepharospasm, an involuntary eyelid twitching. In cosmetic settings, it is widely used for temporarily improving moderate to severe glabellar lines (frown lines), horizontal forehead lines, and lateral canthal lines (crow's feet) on the upper face. The active ingredient, incobotulinumtoxinA, is a highly purified neurotoxin from the Clostridium botulinum Hall strain. Xeomin belongs to the neuromuscular blocking agent or acetylcholine release inhibitor class of drugs.
Chronic sialorrhea often arises from neurological conditions like Parkinson’s disease. Here, a lack of swallowing coordination causes saliva pooling. Xeomin targets the parotid and submandibular glands, thereby reducing saliva volume. This leads to better hygiene and comfort in individuals with this condition. Upper limb spasticity involves an abnormal increase in muscle tone. It is often seen after a stroke or traumatic brain injury. Such spasticity results in restrictive postures, such as a clenched fist or a flexed wrist. By relaxing overactive muscles, Xeomin helps restore some movement and may make daily care easier. Cervical dystonia is a painful condition marked by involuntary contractions that twist the neck into abnormal positions, such as torticollis or retrocollis. Relieving the sternocleidomastoid or trapezius muscles helps return the head toward a neutral position and lessens associated pain. Blepharospasm is characterized by uncontrollable, forceful eyelid closure due to muscle spasms. Xeomin weakens these muscle fibers, helping keep the eye open. For upper facial lines, repetitive muscle contraction like those of the corrugator or frontalis, cause dynamic wrinkles. These wrinkles eventually etch deeply into the skin. Xeomin temporarily halts these contractions. As a result, the skin smooths out, and lines are prevented from deepening.
Xeomin Side Effects
Like all medications, Xeomin can cause some side effects. Common reactions include pain, tenderness, or bruising at the injection site. Drooping of the upper eyelid, double or blurred vision, dry eye, and dry mouth may occur. Some people report eye pain, slowed blinking, fatigue, headache, nausea, neck weakness, flu-like symptoms, pins-and-needles sensations, pain in the affected limb, a change in taste, and thickened saliva. Contact your doctor if these common side effects do not go away or become worse.
Stop using this medicine and tell your doctor right away if you have any serious side effects, including:
• Allergic reactions, such as difficulty breathing, itching, rash, skin blistering, or swelling of the face, mouth, tongue, lips, or throat;
• Breathing problems;
• Frequent falling;
• Heart problems such as chest pain, nausea, vomiting, irregular heartbeat;
• Persistent, difficult, or painful swallowing; speech problems;
dizziness; shortness of breath; vision changes; facial paralysis; or ongoing eye pain or irritation (especially with eye treatments);
• Seizures;
• Urinary tract infection symptoms, such as painful urination, increased frequency, or lower back/flank pain

Xeomin Precautions
Do not use this medicine if you are allergic to incobotulinumtoxinA, any ingredient in Xeomin, or any other medicines. Before starting, let your doctor know if you have any current or past health problems, such as:
• Breathing difficulties;
• Glaucoma;
• Heart disease;
• Neurological conditions such as myasthenia gravis, Lambert-Eaton syndrome, and amyotrophic lateral sclerosis;
• Seizures;
• Swallowing difficulties
It is vital to note that, although Xeomin is injected locally, the toxin can move from the target muscle to other areas. Symptoms of this include generalized muscle weakness, double vision (diplopia), or drooping eyelids (ptosis). More seriously, it can affect muscles needed for speaking, swallowing, and breathing. Inform your doctor immediately if you develop any of these symptoms. Xeomin may cause side effects like muscle weakness, vision changes, dizziness, or drooping eyelids. These effects may impair your ability to do certain activities safely. Do not drive, use machinery, or do hazardous tasks until you know how this medication affects you. Guidelines state that Xeomin should be used in pregnancy only if the expected benefit clearly outweighs the risk to the fetus. It is unknown whether incobotulinumtoxinA is excreted in human milk. Your doctor will balance the treatment needs with potential theoretical risks to the infant. The risk of significant toxin passing into breast milk is considered low. Xeomin is not recommended for children under 2 years or weighing less than 12 kg.
Drugs that affect neuromuscular transmission, such as aminoglycoside antibiotics (e.g., gentamicin or neomycin) or muscle relaxants (e.g., tubocurarine), can greatly increase this toxin's effects. Medications with anticholinergic properties may increase side effects, such as dry mouth or blurred vision, when taken with Xeomin. Tell your doctor before using Xeomin about any other medications, including herbal products and supplements, to check if it is safe for you. Xeomin comes as a powder that your healthcare provider mixes into an injectable solution. A qualified professional injects it into a muscle and decides your dose based on your condition. Store the powder at room temperature. After mixing, it may be refrigerated at 2°C to 8°C for up to 24 hours. Always keep Xeomin out of reach of children.
