Antizol (Fomepizole) Information

Antizol, which contains the active ingredient fomepizole, is an antidote used to treat poisoning from methanol and ethylene glycol poisoning (the main component of antifreeze). These substances can quickly become dangerous in the body. They may cause severe acidosis, blindness, kidney failure, and other life-threatening complications if not treated right away. Antizol is given through a vein in emergency and critical-care settings to stop the body from turning these toxic alcohols into harmful by-products. Early treatment is essential to protect organs and improve outcomes.

The mechanism of action of fomepizole involves blocking the enzyme alcohol dehydrogenase, which usually converts methanol and ethylene glycol into toxic acids. By inhibiting this enzyme, Antizol prevents the formation of toxic metabolites, reduces organ damage, and allows the body to eliminate the poisonous alcohols more safely. In severe cases, hemodialysis may be used to remove the substances faster. Because of its predictable and reliable action, fomepizole is the preferred first-line treatment for toxic alcohol ingestion.

Antizol Side Effects

Antizol is generally well-tolerated, but some common side effects may occur. Common side effects include headache, nausea, dizziness, mild drowsiness, and a metallic or unpleasant taste. Some people may notice redness, itching, or pain at the injection site.

More serious side effects can occur, leading to significant monitoring protocols. Doctors will monitor for these side effects and may adjust the infusion rate or discontinue therapy if a severe hypersensitivity reaction occurs:
• Liver enzyme elevations, which may require monitoring
• Low blood pressure or rapid heart rate, especially with rapid infusion
• Severe dizziness
• Signs of allergic reactions, including rash, swelling of the face or throat, difficulty breathing

Antizol Precautions

Do not use Antizol if you are allergic to fomepizole or any of the ingredients in the drug. Before treatment, your doctor will evaluate the patient’s history when possible. However, treatment is often started immediately because toxic alcohol poisoning is a medical emergency. People with liver disease may require closer monitoring because fomepizole can increase liver enzyme levels.

Tell your doctor about all medications you take, including prescription drugs, over-the-counter medicines, herbs, and supplements. Alcohol and fomepizole use the same enzyme system, so if alcohol is present in the body, the medication may stay in the system longer, and dosing intervals may need to be adjusted. Significant drug interactions are rare because Antizol is used in emergencies. Inform your doctor if you are pregnant or planning to become pregnant. Fomepizole is considered safer than leaving methanol or ethylene glycol poisoning untreated, as the poisoning itself is far more dangerous to both mother and fetus. If you are breastfeeding, discuss this with your doctor, as it is not fully known whether the medication enters breast milk. Treatment should not be delayed because toxic alcohol poisoning is life-threatening. Antizol is administered intravenously in a hospital by trained doctors. Dosing is based on body weight and given at scheduled intervals, with adjustments made if the patient is undergoing hemodialysis. After the initial dose, maintenance doses are administered at regular intervals, typically every 12 hours. Continuous monitoring of blood pH, electrolytes, toxic alcohol levels, and kidney function is essential. Do not attempt to self-administer this medication.

Your doctor will decide when it is safe to stop treatment based on toxic alcohol levels, improvement in acidosis, and overall clinical stability. The length of therapy depends on how much poisonous alcohol is in the body, how quickly levels fall, and whether dialysis is needed. Antizol is stored and handled in accordance with hospital standards and kept out of reach of unauthorized personnel.
