Bosulif Information

Bosulif (Bosutinib Monohydrate) is used to treat the chronic phase of Philadelphia chromosome-positive chronic myeloid leukemia (CML) in newly diagnosed individuals. It is also used in previously treated adults to treat chronic, accelerated, or blast phase who are resistant or intolerant to prior therapies like imatinib or dasatinib. It is also used in children aged 1 year and older with chronic phase Philadelphia chromosome-positive chronic myeloid leukemia, either newly diagnosed or previously treated. Chronic myeloid leukemia is a type of cancer that originates in the bone marrow, where a genetic translocation creates the abnormal BCR-ABL gene, leading to the overproduction of dysfunctional white blood cells that eventually crowd out healthy blood components.

This medication contains the active ingredient bosutinib monohydrate, a tyrosine kinase inhibitor (TKI). Its primary mechanism is to bind and inhibit the BCR-ABL protein and Src-family kinases, thereby preventing cellular proliferation and halting the signaling pathways that drive the out-of-control division of leukemia cells. This mechanism helps manage the disease by reducing tumor burden, restoring normal blood cell counts, and preventing leukemia from progressing to more aggressive phases. Interestingly, its dual-action targeting of Src kinases may suggest a slightly different resistance profile compared to first-generation inhibitors, though it remains ineffective against specific mutations.

Do not take this medication if you are allergic to bosutinib, other ingredients in Bosulif, or any other medicines. Before you start taking this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Active infection;
• Anemia;
• Bone problems;
• Diabetes;
• Electrolyte imbalance;
• Gastrointestinal problems;
• Heart disease, such as heart failure;
• Hepatitis B;
• Kidney disease;
• Liver disease;
• Pancreas problems, such as pancreatitis

Bosulif can cause a significant drop in blood counts (neutropenia, thrombocytopenia, and anemia). Your doctor may order blood tests regularly to monitor the levels of specific blood cell types. Do not drive or use machinery until you know how this medication affects you, as it can cause drowsiness and body weakness. Grapefruits block the enzymes that break down this medication, potentially leading to toxic levels in the body. Therefore, it's advised to avoid taking grapefruit or grapefruit-containing drinks while using this medication. This medication can make you more susceptible to infections. Therefore, avoid contact with individuals who are infected with measles, chickenpox, or the flu. It is generally advised to avoid using this medication during pregnancy or while breastfeeding, as it can cause harm to the fetus and the breastfed infant. Both men and women of reproductive age are typically advised to use effective contraception during treatment and for at least two weeks after the final dose. If you get pregnant while taking this medication, inform your doctor immediately. This medication is not recommended for use in children younger than 6 years.

It is important to avoid CYP3A inhibitors (such as clarithromycin or certain antifungals) and inducers (such as St. John’s Wort or rifampin). These can either cause the drug to build up to toxic levels or make it lose its effectiveness entirely. Also, before using this medication, inform your doctor about any other medications you are taking, including herbal medicines and supplements, to determine whether it is safe to use with Bosulif. Caution is advised when using this medication in people of Asian descent, as they may be more sensitive to its side effects. Swallow Bosulif tablets whole once daily during a meal. Taking this medication with food significantly improves the absorption of its active ingredient. You should avoid crushing, chewing, or breaking the tablets, as this alters the intended delivery of the drug. Your doctor will determine the specific dosage based on whether you have a new diagnosis or a resistance to previous leukemia treatments. Always take the dose at approximately the same time each day to maintain steady levels in your bloodstream. Store this medication at room temperature, protect it from light and moisture, and keep it out of the reach of children.
