Cortrosyn Information

Cortrosyn (cosyntropin) is a synthetic derivative of the naturally occurring adrenocorticotropic hormone (ACTH). It is used strictly as a diagnostic agent in the ACTH stimulation test to screen for adrenocortical insufficiency—a condition where the adrenal glands are underactive and fail to produce enough cortisol.

Cortrosyn works by directly stimulating the adrenal glands to release cortisol, allowing doctors to evaluate how well these glands function. Once injected into a vein or muscle, the synthetic hormone travels through the bloodstream and binds to specific receptors on the adrenal glands. This binding triggers the adrenal cells to immediately synthesize and secrete cortisol into the blood. By drawing blood before the injection and again 30 to 60 minutes afterward, clinicians observe the change in hormone levels: a robust rise indicates healthy, functioning adrenal glands, while a minimal or absent response confirms adrenal insufficiency.

Cortrosyn Side Effects

Because Cortrosyn is used as a one-time diagnostic injection rather than an ongoing treatment, side effects are typically minimal. You may experience mild redness, swelling, or a slight rash at the injection site. Other rare but possible side effects include a temporary fast or slow heart rate, lightheadedness, or mild swelling in the limbs.

• Corticosteroids: Taking prescription steroid medications (such as prednisone, hydrocortisone, or dexamethasone) can suppress your natural adrenal function or interfere directly with the test's blood analysis. Your doctor will provide specific instructions on how and when to temporarily pause these medications prior to your test.

• Diuretics: Cosyntropin can occasionally accentuate the electrolyte loss (particularly potassium depletion) associated with concurrent diuretic use. While the risk is very low with a single diagnostic dose, your care team should monitor your electrolyte levels if you take "water pills."

There are no adequate, well-controlled clinical studies on Cortrosyn use in pregnant women; it should only be administered during pregnancy if the diagnostic benefit clearly outweighs any potential fetal risk. It is currently unknown whether cosyntropin is excreted in human breast milk. However, because cosyntropin is a peptide molecule, it is highly likely to be degraded in the infant's digestive tract if trace amounts are ingested. Clinicians advise exercising caution and discussing the risks and benefits before receiving this drug while pregnant or breastfeeding. Prior to reconstitution, store the medication at room temperature, protect it from light and moisture, and keep it safely out of the reach of children.
