Gleevec (Imatinib) Information
Gleevec, containing the active ingredient imatinib mesylate, is a landmark targeted therapy classified as a tyrosine kinase inhibitor (TKI). It is primarily used to treat Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) and acute lymphoblastic leukemia (ALL). It is also highly effective in treating gastrointestinal stromal tumors (GIST) and certain other rare blood disorders such as myelodysplastic or myeloproliferative diseases. Often referred to as a "magic bullet" in oncology, Gleevec changed the landscape of cancer care by targeting specific proteins rather than attacking all rapidly dividing cells. The medication is taken by mouth as a tablet, typically once or twice daily.
The mechanism of action involves the precise blocking of the BCR-ABL tyrosine kinase, an abnormal protein produced by the Philadelphia chromosome that signals leukemia cells to multiply uncontrollably. In GIST, it targets the KIT and PDGF receptor tyrosine kinases. By binding to the "pocket" of these proteins, Gleevec prevents them from receiving the energy they need to transmit growth signals. This action halts the production of cancerous cells. It induces cell death, allowing for the restoration of normal blood cell production.
Gleevec Side Effects
Gleevec is generally well-tolerated, but some common side effects may occur as your body adjusts to the medication. Commonly reported effects include nausea, vomiting, diarrhea, muscle cramps, joint pain, and a skin rash. A common side effect of Gleevec treatment is fluid retention, which often presents as puffiness or swelling around the eyes (periorbital edema) or in the lower legs.
More serious side effects can occur and require immediate medical attention.
Stop taking the medication and seek help if you experience:
• Gastrointestinal Perforation: Severe, sudden abdominal pain or swelling.
• Heart Problems: Chest pain, palpitations, or signs of congestive heart failure.
• Liver Toxicity: Yellowing of the skin or eyes (jaundice), dark urine, or severe pain in the upper right stomach.
• Low Blood Cell Counts: Fever, chills, or extreme weakness, indicating a severe drop in white or red blood cells.
• Severe Bleeding: Blood in the stool or urine, unusual bruising, or persistent nosebleeds, as Gleevec can affect platelet function and the gastrointestinal lining.
• Severe Fluid Retention: Sudden weight gain, shortness of breath, or swelling of the abdomen (ascites) or chest area.
Gleevec Precautions
Do not use Gleevec if you have a known allergy to imatinib or any other ingredients in the tablet. Before starting treatment, inform your doctor of your complete medical history, especially if you have a history of liver, kidney, or heart disease, or if you have had your thyroid removed. Because the drug can cause significant fluid retention, patients with underlying heart or lung conditions must be monitored closely for signs of strain.
Tell your doctor about all medications you take, as Gleevec has a wide array of drug interactions. Its levels can be dangerously increased by strong CYP3A4 inhibitors, such as certain antifungals (e.g., ketoconazole) and antibiotics (e.g., erythromycin). Conversely, medications like St. John's Wort or certain anti-seizure drugs (e.g., phenytoin) can make Gleevec less effective. You must avoid grapefruit and grapefruit juice, as they can interfere with the metabolism of the drug. Additionally, tell your doctor if you take blood thinners like warfarin or over-the-counter NSAIDs, as these can increase the risk of gastrointestinal bleeding.
Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Gleevec can cause fetal harm and is generally not recommended during pregnancy; women of childbearing age should use highly effective contraception during treatment and for at least 14 days after the final dose. Avoid breastfeeding while taking Gleevec and for at least one month after the final dose, as the medication passes into breast milk and could affect the nursing infant.
Take Gleevec exactly as prescribed, ideally with a large glass of water and a meal to minimize gastrointestinal irritation. If you have trouble swallowing the tablets, dissolve them in a glass of water or apple juice. Do not miss doses, as consistent levels of the drug are required to keep the cancer in remission. Store the tablets at room temperature in a dry place.
