Imbruvica (Ibrutinib) Information
Imbruvica, which contains the active ingredient ibrutinib, is a first-in-class targeted therapy known as a Bruton’s tyrosine kinase (BTK) inhibitor. It is primarily used to treat various blood cancers, including chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), and Waldenström’s macroglobulinemia. It is also used to treat chronic graft-versus-host disease (cGvHD) in patients who have failed previous systemic therapies. Unlike traditional chemotherapy, Imbruvica targets the signaling pathways that enable cancer cells to survive. It is taken by mouth, typically as a tablet or capsule once daily.
The mechanism of action involves the permanent binding of ibrutinib to the Bruton’s tyrosine kinase protein. This protein is a key component of the B-cell receptor signaling pathway, which malignant B-cells rely on for growth, migration, and survival. By blocking BTK, Imbruvica disrupts these survival signals, leading to decreased cancer cell proliferation and their clearance from the lymph nodes and bone marrow.
Imbruvica Side Effects
Imbruvica is generally well-tolerated, but some common side effects may occur as your body adjusts to the medication. Commonly reported effects include diarrhea, fatigue, muscle or joint pain, and upper respiratory tract infections. Other frequent effects include nausea, constipation, mouth sores, and a mild skin rash. You may also notice your fingernails becoming brittle or thin during treatment.
More serious side effects can occur and require immediate medical attention. Stop taking the medication and seek help if you experience:
• Heart Rhythm Problems: A racing, pounding, or irregular heartbeat, or feeling lightheaded (Atrial fibrillation or flutter).
• Hypertension: New or worsening high blood pressure that requires monitoring or treatment.
• Severe Bleeding: Significant bruising, blood in the stool or urine, or any bleeding that will not stop (Imbruvica can increase the risk of hemorrhage).
• Severe Infections: Fever, chills, or persistent cough, as the medication can lower your body’s ability to fight off bacteria and viruses.
• Tumor Lysis Syndrome: Confusion, muscle spasms, or decreased urination, caused by the rapid breakdown of cancer cells.
Imbruvica Precautions
Do not use Imbruvica if you have a known allergy to ibrutinib or any of the inactive ingredients in the capsule or tablet. Before starting treatment, inform your doctor of your complete medical history, especially if you have liver disease, a history of heart rhythm problems (like atrial fibrillation), recent or planned surgery, or a history of bleeding disorders. Because Imbruvica can increase the risk of bleeding during and after surgery, your doctor may instruct you to stop taking it for 3 to 7 days before and after any planned surgical or dental procedure.
Tell your doctor about all medications you take, as Imbruvica has several high-risk drug interactions. Its levels in the blood can be significantly increased by strong CYP3A inhibitors, such as certain antifungals (ketoconazole, itraconazole), certain antibiotics (clarithromycin), and some heart medications (diltiazem, verapamil). You must avoid grapefruit, grapefruit juice, and Seville oranges, as they can interfere with the breakdown of the drug and lead to toxicity. Additionally, tell your doctor if you take blood thinners (like warfarin, aspirin, or ibuprofen), as these can further increase your risk of severe bleeding.
Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Imbruvica can cause significant harm to a developing fetus and should not be used during pregnancy. Women of childbearing potential should use effective birth control during treatment and for one month after the final dose; men should also use effective contraception during this time. It is recommended that you do not breastfeed while taking Imbruvica and for one week after the last dose.
Take Imbruvica exactly as prescribed, at the same time each day with a glass of water. Swallow the capsules or tablets whole; do not open, break, or chew them. If you miss a dose by more than 12 hours, skip it and take the next dose as scheduled. Store the medication at room temperature in its original container, away from moisture and heat.
