Lorbrena Information
Lorbrena (Lorlatinib) is a potent, third-generation tyrosine kinase inhibitor, specifically engineered to address a significant hurdle in cancer treatment: cancer cells' ability to mutate and become resistant to earlier drugs. It is primarily used to treat adults with non-small cell lung cancer (NSCLC) caused by an abnormal anaplastic lymphoma kinase (ALK) gene, and that has spread to other parts of the body. It has also been found effective as a first-line treatment of adults with newly diagnosed non-small cell lung cancer (NSCLC) that is caused by an abnormal anaplastic lymphoma kinase (ALK) gene.
Its active ingredient, lorlatinib, is a third-generation tyrosine kinase inhibitor that functions as a highly targeted therapy. It works by binding to and blocking the abnormal anaplastic lymphoma kinase protein, thereby turning off the internal signaling pathways that drive cancer cell growth and division. To understand why this matters, Non-small cell lung cancer is a widespread form of lung cancer, and in the anaplastic lymphoma kinase-positive subtype, a genetic rearrangement creates a fusion protein that acts as a permanent on switch for cell proliferation. Lorlatinib appears to be particularly effective because it was specifically designed to cross the blood-brain barrier and overcome resistance mutations that often hinder earlier drugs. Suppressing these drivers helps shrink existing tumors and, perhaps most importantly, prevents the spread of cancer to the brain (a frequent complication in these patients). Ultimately, this approach helps patients live considerably longer without their disease progressing, providing a critical tool against what is otherwise a highly aggressive malignancy.
Recent long-term data from clinical studies (with 5-year follow-up) suggest that Lorbrena provides unprecedented disease control. It appears to be exceptionally effective at preventing cancer from spreading to the brain, with a roughly 94% reduction in the risk of intracranial progression.
Lorbrena Side Effects
Like all medications, lorlatinib can cause some side effects. Common side effects of this medication include swelling in your arms, legs, hands, and feet (edema), numbness and tingling feeling in your joints or arms and legs (peripheral neuropathy), weight gain, problems with thinking such as forgetfulness or confusion, tiredness (fatigue), difficulty breathing, pain in your joints, diarrhea, changes in mood such as depression and irritability, high cholesterol and triglyceride levels in the blood, and cough. Contact your doctor if these common side effects persist or worsen.
Stop taking this medicine and contact your doctor immediately if you experience any serious side effects, including the following:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, swelling of the face, mouth, tongue, lips, or throat;
• Central nervous system effects such as confusion, mood changes including depression and suicidal thoughts, hallucinations, seizures;
• Heart problems with symptoms like dizziness, fainting spells, or abnormal heartbeats;
• High blood pressure with symptoms like headaches, blurred vision, palpitations;
• High blood sugar with symptoms like feeling very thirsty, needing to urinate more than usual, or feeling very hungry, sick to your stomach, weak or tired, or confused;
• Increases in the cholesterol and triglyceride (lipid) levels in your blood;
• Liver problems such as yellow eyes and skin, persistent nausea and vomiting;
• Lung problems with symptoms like trouble breathing, shortness of breath, cough, or fever

Lorbrena Precautions
Do not take this medication if you are allergic to lorlatinib, other ingredients in Lorbrena, or any other medicines. Before you start taking this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Depression;
• Diabetes;
• Heart disease;
• High blood pressure;
• Kidney disease;
• Liver disease;
• Lung problems;
• Seizure disorders
Your doctor may request regular blood tests before and during treatment with this medication to monitor its effects. If levels of certain substances, such as lipids, become too high, you may need lipid-lowering medications. Lorlatinib may harm an unborn baby, so it's advised to avoid pregnancy during treatment and for a period after stopping the drug. It should not be used during breastfeeding either, as it could pass into breast milk. Do not breastfeed during treatment with this medication and for 7 days after the final dose. Women of reproductive potential must use highly effective non-hormonal contraception (like condoms or a copper intrauterine device) during treatment with Lorbrena and for 6 months after the last dose. Likewise, men with female partners of reproductive potential are advised to use effective contraception during treatment and for 3 months after the final dose. There may be decreased fertility in males due to Lorbrena use. It could affect your ability to have children. Talk to your healthcare provider if you have concerns about your fertility.
Taking Lorbrena with some strong liver enzyme inducers, such as rifampin and carbamazepine, is strictly not recommended. In clinical trials, healthy individuals exposed to both medications saw a dramatic spike in liver enzymes (ALT/AST), sometimes reaching life-threatening levels within just a few days. Therefore, before using this medication, inform your doctor about any other medications you are taking, including herbal medicines and supplements, to determine whether it is safe to use with Lorbrena. Take Lorbrena tablets once daily, with or without food. Swallow the tablet whole; do not chew or crush it. The typical starting dose is 100 mg per day, but your doctor may adjust it based on your response and side effects. Check out the leaflet in the medication pack for more usage instructions. Store this medication at room temperature, protect it from light and moisture, and keep it out of the reach of children.
