What is Narcan (Naloxone) Used For?

Narcan (Naloxone) is an opioid antagonist, specifically used for the emergency treatment of a known or suspected opioid overdose, particularly when a person shows signs of respiratory or central nervous system depression. These signs often include extreme sleepiness, an inability to speak or be awakened, very small or pinpoint pupils, and breathing that has become dangerously slow or has stopped entirely. It is designed for immediate use in any place where opioids, such as fentanyl, heroin, oxycodone, or morphine, might be present.

Naloxone works by interacting with specific parts of the nervous system called opioid receptors. When an individual overdoses on opioids, these opioids attach to these receptors and cause life-threatening symptoms, such as the suppression of the drive to breathe. Naloxone is a competitive antagonist, which means it has a high affinity for these same receptor sites and will compete with the opioids to occupy them. When Narcan is administered, it binds to these receptors and displaces the opioid molecules already attached. By physically occupying the receptor sites and blocking the opioids from binding, naloxone reverses and blocks the effects of the opioids on the body.

Naloxone has a rapid onset of action, with effects usually seen within 2 minutes of administration. However, its duration of action is typically between 30 and 90 minutes, which is often shorter than the duration of the opioids themselves. As a result, individuals treated with this medication must be monitored closely, and emergency medical assistance (911) must be called immediately, as the effects of the overdose may return once the naloxone is metabolized and wears off.

This medication should be administered as stated in the medication leaflet. For complete information about its use, view the full product monograph.

What Side Effects Can Narcan or Generic Naloxone Cause?

Naloxone nasal spray is generally considered a safe medication, and side effects are relatively rare when given to someone without opioids in their system.

Common side effects include nasal dryness, nasal swelling, and congestion. Other frequently reported mild side effects include headaches, musculoskeletal pain, and general body aches.

A significant concern with the use of this medication is the precipitation of sudden and severe opioid withdrawal in individuals who are physically dependent on opioids. This reaction occurs because the medicine rapidly removes opioids from the brain's receptors. These often involve symptoms such as nausea, vomiting, diarrhea, sweating, fever, rapid heart rate, nervousness, restlessness, irritability, aggressive behavior, piloerection (goosebumps), yawning, runny nose, sneezing, and tremors. Contact your doctor if these side effects persist or worsen. However, withdrawal symptoms in neonates with opioid dependence, such as seizures, crying more than usual, and increased reflexes, usually require urgent medical attention.

Serious side effects may include:
• Allergic reactions such as hives, swelling of the face, lips, tongue, or throat, or difficulty breathing or swallowing
• Respiratory problems such as pulmonary edema with symptoms such as severe shortness of breath, wheezing, or frothy sputum, likely triggered by a massive sympathetic surge;
• Severe cardiovascular reactions such as an abnormally fast heartbeat, fainting, or severe high blood pressure;

Brain injury, cognitive impairment, or paresthesia occurring after an overdose are usually the result of oxygen deprivation (hypoxia) from the overdose itself before treatment, rather than a direct side effect of naloxone.

Seek immediate medical attention if you experience any of these serious side effects after you’ve been administered this medication. Also, inform your doctor about any other side effects you notice that are not listed here.

Warnings

Do not use this medication if you are allergic to naloxone or any other ingredients in Narcan. Caution is advised when using this medication in individuals with some specific medical conditions such as:
• Heart or circulation problems;
• Kidney disease;
• Liver disease;
• Seizure disorder;

Narcan nasal spray is strictly for intranasal use. It must not be used as a substitute for professional emergency medical care; it is critical to call 911 or local emergency services immediately after administering the first dose. It is worth noting that some specific types of opioids, like buprenorphine or high-potency synthetic opioids like fentanyl, bind very strongly to receptors; they often require higher or more frequent doses of naloxone to achieve a complete and lasting reversal of respiratory depression. Caution is advised when administering naloxone to individuals taking drugs with potential adverse effects on the heart (such as sympathomimetics or certain antihypertensives), as additive heart stress may occur during abrupt opioid withdrawal. Be sure to tell your healthcare provider about the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.

It is known that naloxone, when used in pregnant women, can cross the placenta and can trigger withdrawal symptoms in the unborn baby. However, because an overdose is life-threatening to both the mother and fetus, naloxone should still be administered immediately in an emergency, followed by evaluation of the fetus by a healthcare provider for signs of distress. Studies in breastfeeding mothers have also shown that naloxone does not interfere with the specific hormones that control milk production. Store this medication at room temperature and keep away from children and pets.
