Optimmune (Cyclosporin) Information
Optimmune, which contains the active ingredient cyclosporin, is a prescription
veterinary medication. It is specifically indicated for the management of chronic keratoconjunctivitis sicca (KCS), commonly known as dry eye, and chronic superficial keratitis (CSK), also known as pannus, in dogs. The medication is a sterile, preservative-free ointment. Although initial improvement in tear production can be seen within 10 days, it may take up to 6 weeks of consistent treatment to achieve the maximum effect.
The mechanism of action involves inhibiting the activation of T-cells and other inflammatory cells in the eye. In dogs with KCS or CSK, the immune system mistakenly attacks the tear glands, leading to inflammation and reduced tear production. This is a lifelong condition, and this medication manages the condition; it's not a cure. Cyclosporine is an immunosuppressant and immunomodulator. By decreasing immune-mediated swelling, the medication allows the tear glands to recover and increase the production of natural tears.
Optimmune Side Effects
Most dogs tolerate Optimmune well, but mild, localized reactions are common when starting treatment. During the first few days, you may notice slight eye irritation, which can manifest as redness, watery eyes, or blepharospasm (excessive squinting or blinking). Some pets also experience a temporary stinging or burning sensation upon application, or mild hair loss in the skin directly surrounding the treated eye, which is often caused by excess ointment that has overflowed onto the fur. To prevent this, gently wipe away excess ointment from the eyelids/fur with a clean, soft tissue after application. If localized irritation or eyelid swelling persists for more than 7 days, treatment may need to be discontinued.
More serious side effects can occur and require immediate veterinary attention. Stop administering the medication and contact your veterinarian if your pet shows signs of:
• Severe allergic reaction: difficulty breathing, swelling of the throat or face, or hives.
• Signs of potential immune suppression: fever or unusual bruising.
• Unusual systemic reactions: extreme lethargy, vomiting, inappetence
Optimmune Precautions
Before starting your dog on Optimmune, inform your veterinarian if your dog has a history of viral or fungal eye infections, as immunosuppressants can mask or worsen these conditions. You should also disclose if the pet has a history of immune system disorders or is currently taking other immunosuppressant drugs. The safety of Optimmune has not been established in puppies or breeding dogs.
There are no documented major drug or food interactions for Optimmune in dogs. However, it is essential to tell your veterinarian about all other medications, vitamins, or supplements your pet is receiving. If you are administering multiple eye medications, wait at least 5 to 10 minutes between products to ensure each is absorbed properly. As a general rule, eye drops should be applied before ointments.

Apply Optimmune exactly as your veterinarian tells you. To apply Optimmune, first wash your hands and put on gloves. Gently pull your dog's lower eyelid down to create a small pocket, then apply the prescribed amount of ointment (typically a 1/2-inch ribbon) directly into the eye or the conjunctival sac. Do not allow the tube tip to come into contact with any surface, including your hands or your pet's face, as this can contaminate the sterile ointment. Store the tube at room temperature (between 2°C and 25°C or 36°F and 77°F) in a cool, dry place away from direct sunlight and moisture. Handle the metal tube gently to prevent cracking or leaking, and never fold it. Squeeze it from the bottom. Any ointment remaining one month after opening the tube should be discarded. Always keep this medication in a secure location out of the reach of children and pets to prevent accidental ingestion.
