What is Promacta (Eltrombopag Olamine) Used For?
Promacta (Eltrombopag Olamine) is a thrombopoietin receptor agonist, primarily used to treat conditions characterized by low blood platelet counts, a state known as thrombocytopenia. It is typically
used for adults and children one year of age and older with chronic immune thrombocytopenia when other treatments like corticosteroids or surgery to remove the spleen have not been effective, and the risk of bleeding is high.
It is also used in individuals with chronic hepatitis C to increase platelet counts enough to allow them to start and stay on interferon-based antiviral therapies. It is also used for patients with severe aplastic anemia, either as a first-line treatment in combination with immunosuppressive therapy for adults and children two years and older, or as a stand-alone treatment for those whose condition has not responded to previous immunosuppressive medications.
Promacta acts directly on the bone marrow where blood cells form. Platelets normally form when a natural body messenger binds to receptor sites on bone marrow progenitor cells. Promacta binds to the same receptor sites at a different location. This attachment triggers signaling within the cell, directing the bone marrow to produce, mature, and release more platelets into the bloodstream. An increased platelet count helps the blood form clots more effectively, reducing the risk of easy bruising and spontaneous bleeding.

Warnings
Do not use this medication if you have an allergic reaction to eltrombopag olamine, or other ingredients in Promacta. Use this medication with caution in individuals with some specific conditions such as:
• Blood cancer;
• Blood clotting or bleeding disorders;
• Eye problems such as cataract;
• Having had surgical removal of the spleen;
• Liver problems;
This medication can cause severe and potentially life-threatening liver injury. As a result, liver function tests must be done before starting treatment, every two weeks during dose adjustments, and monthly once a stable dose is reached. People of East Asian or Southeast Asian ancestry (such as Chinese, Japanese, Taiwanese, or Korean descent) metabolize eltrombopag differently and can potentially have higher medication levels despite the standard dose. These individuals require a lower starting dose.
In individuals with chronic hepatitis C, taking this medication with interferon and ribavirin increases the risk of hepatic decompensation (liver failure) and death. Eltrombopag binds to minerals like calcium, aluminum, magnesium, iron, selenium, and zinc found in foods (such as dairy), mineral supplements, and antacids. This significantly reduces its absorption in the body. As a result, it is typically recommended to take eltrombopag at least 2 hours before or 4 hours after these mineral supplements. Significant interactions have also been recorded with the use of this medication with blood thinners and cholesterol drugs like statins. Therefore, before using this medication, tell your doctor about any other medications you are currently taking, including herbal medications and supplements, so they can determine whether it is safe to use with Promacta.
There aren't adequate human data to confirm the safety of this medication during pregnancy, but animal studies show fetal harm and embryo-fetal death. Women of reproductive age taking this medication should use effective birth control while taking the drug and continue for at least 7 days after stopping it. It is also unknown whether eltrombopag passes into breast milk. Still, due to risks of severe liver damage and abnormal cell growth in a breastfed infant, breastfeeding is not recommended while using this medication. Store this medicine at room temperature, and keep it out of reach of children and pets.
