Zofran Information
Zofran (Ondansetron Hydrochloride Dihydrate) is used to prevent and treat nausea and vomiting induced by chemotherapy medications. It is also used to prevent nausea and vomiting caused by radiotherapy in individuals undergoing full-body or localized radiation. It is also used before or after surgery to counter the effects of anesthesia, such as nausea and vomiting. The active ingredient is Ondansetron hydrochloride dihydrate, a 5-HT3 receptor antagonist (also known as a serotonin blocker). Ondansetron works by blocking the action of serotonin, a natural chemical in the body.
During treatments with chemotherapy or anaesthetic agents during surgery, the body often releases a surge of serotonin in the small intestine and the brain. This serotonin normally binds to 5-HT3 receptors, which then signal the brain’s vomiting center. Ondansetron selectively blocks the action of serotonin (5-hydroxytryptamine) at the 5-HT3 receptors located both in the gastrointestinal tract and the chemoreceptor trigger zone in the brain. When these receptors are blocked, the vomit signal cannot be transmitted to the brain's vomiting center, thereby preventing the physical urge to throw up. This allows patients to undergo life-saving cancer treatments or recover from the side effects of general anesthesia without the debilitating dehydration and physical stress that chronic vomiting causes. While ondansetron appears highly effective for these specific triggers, it is likely less helpful for motion sickness, which involves a different pathway in the inner ear.
Ondansetron has also been shown to offer some benefits in the treatment of some other conditions, which include severe morning sickness in pregnant women, migraine-associated nausea, body itching caused by conditions such as kidney disease, gallbladder disease, and anaesthetic agents. It may also be used to treat nausea caused by gastrointestinal conditions such as gastroenteritis and gastroparesis.
Zofran Side Effects
Like other medications, ondansetron can cause some side effects. Common side effects of this medication include headache, flushing or warmth, hiccups, constipation, diarrhea, and unusual tiredness or weakness. Contact your doctor if these mild side effects persist or worsen.
Stop taking this medicine and contact your doctor immediately if you experience any serious side effects, including the following:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, especially on the hands and feet, swelling of the face, mouth, tongue, lips, or throat;
• Electrolyte imbalances with symptoms like weakness, fatigue, muscle cramps, irregular heartbeat;
• Eye problems such as blurry vision, visual loss;
• Heart problems such as chest pain, chest discomfort, high blood pressure, prolongation of the QT interval, and shortness of breath;
• Liver problems with symptoms like nausea, vomiting, diarrhea, loss of appetite, weight loss, and yellowing of the skin or whites of the eyes;
• Movement disorders such as loss of coordination or balance, speech or limb movements, muscle spasms;
• Seizures;
• Serotonin syndrome with symptoms such as confusion, mood changes, decreased coordination, hallucinations, fever, sweating

Zofran Precautions
Do not take this medication if you are allergic to ondansetron, other ingredients in Zofran, or any other medicines. Before you start taking this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Abdominal problems such as paralytic ileus, intestinal obstruction;
• Diabetes;
• Electrolyte imbalance, such as low levels of potassium in the blood;
• Heart disease, including a family history of heart disease or sudden cardiac death;
• High cholesterol levels;
• Liver disease;
• Low blood pressure;
• Nutritional deficiencies;
• Stroke
While ondansetron can be used for severe morning sickness (hyperemesis gravidarum) when other treatments fail, some studies have suggested a very slight increase in the risk of oral clefts (like a cleft palate) or heart defects if taken during the first trimester. Because of this, many practitioners may suggest waiting until the second trimester or trying other options first. Consult your doctor before using this medication during pregnancy or while breastfeeding. Consult your doctor if you become pregnant while using this medication. Safety and effectiveness for chemotherapy-induced nausea have not been established in children under 4, though it is used in younger children for other conditions under strict medical supervision. It should be used with caution in older adults (65 years and above) as they may be more prone to its side effects.
Taking Zofran with drugs like amiodarone, certain antipsychotics, or some antibiotics (like erythromycin) can compound the risk of heart rhythm issues. Apomorphine is a medication used for Parkinson’s disease. Combining it with Zofran is strictly contraindicated because it can cause a severe drop in blood pressure and loss of consciousness. Because Zofran is an antiemetic, it may mask progressive ileus or gastric distension in patients following abdominal surgery. Therefore, before using this medication, it is important to inform your doctor about any other medications you are currently taking, including herbal products and supplements, to determine whether ondansetron hydrochloride is safe to use with them. The risk of Serotonin Syndrome increases when Zofran is taken with other serotonergic drugs, such as SSRIs (e.g., Prozac, Zoloft) or SNRIs (e.g., Cymbalta, Effexor). Swallow the standard tablets whole with a glass of water, either with or without food. If you use the orally disintegrating tablets, you should place the pill on your tongue and let it dissolve completely before swallowing. Patients with Phenylketonuria (PKU) should be aware that the orally disintegrating tablets (ODT) may contain phenylalanine. Your doctor will determine the specific dose based on your body weight and the severity of your nausea. You must follow the exact timing on your prescription label, as taking the dose 30 to 60 minutes before a known trigger yields the best results. Always store the tablets in their original packaging at room temperature to protect them from moisture and light.
