Zometa Information

Zometa (Zoledronic Acid) is used to treat several conditions related to bone health and mineral metabolism, particularly in cancer patients or those with metabolic bone diseases. It is used to treat severely elevated blood calcium levels caused by cancer. It is also used in the treatment of multiple myeloma, a type of blood cancer that affects plasma cells in the bone marrow and often destroys bone tissue. It is also used in preventing bone fractures due to bone metastases from cancers such as breast, prostate, or lung cancer.

Zoledronic acid is used in the treatment and prevention of osteoporosis in men, postmenopausal women, and individuals on long-term steroids. It is used in these individuals to increase bone mass and reduce the risk of fractures. Zoledronic acid may also be used to treat Paget’s disease of the bone, a condition in which the body’s normal bone-recycling process is overactive and disorganized. Some hormone reduction therapies used in some cancers, such as prostate cancer and breast cancer, have been found to increase the risk of bone fractures. Zoledronic acid has been proven effective in reducing the risk of bone fractures associated with these therapies. The active substance is zoledronic acid (sometimes referred to as zoledronate), which belongs to a group of medications called bisphosphonates, specifically the nitrogen-containing, third-generation subclass, which are the most potent.

Zoledronic acid appears to have a high affinity for bone minerals. It binds to the bone surface and inhibits osteoclast activity (the cells that break down bone). Tumors like multiple myeloma can drive these osteoclastic cells into overdrive, creating weak spots or lytic lesions that make bones fragile and lead to excess calcium release into the blood. It also reduces the release of calcium from bones into the bloodstream, which is beneficial in conditions such as hypercalcemia of malignancy. Essentially, in each of these conditions, zoledronic acid targets the underlying issue of excessive bone resorption, either due to disease (like cancer metastasis) or metabolic changes (like osteoporosis), offering crucial support to maintain bone health and prevent complications.

Zometa Side Effects

Like other medications, zoledronic acid can cause some side effects. Common side effects of this medication include bone or joint pain, drowsiness, eye redness or excessive tearing, fever, fatigue, loss of appetite, nausea, redness or swelling at the infusion site, sleepiness, thirst, and vomiting. Contact your doctor if these mild side effects persist or worsen.

Stop taking this medicine and contact your doctor immediately if you experience any serious side effects, including the following:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, especially on the hands and feet, swelling of the face, mouth, tongue, lips, or throat;
• Chest pain, irregular heartbeat;
• Flu-like symptoms such as fever, chills, bone pain, or muscle and joint pain;
• High blood calcium levels with symptoms such as loss of appetite, nausea, vomiting, constipation, or stomach pain;
• Irregular heartbeat (Atrial fibrillation);
• Jaw problems such as numbness or feeling of heaviness in the jaw, poor healing of the gums, loose teeth, exposed bone in the mouth, swelling or gum infections, bad breath, or pain in the mouth, teeth, or jaw;
• Kidney function with symptoms such as a change in urine colour, little or no urine, lower back pain, fatigue, nausea, loss of appetite;
• Persistent eye pain, redness or swelling, vision changes, or sensitivity to light

Zometa Precautions

Do not take this medication if you are allergic to zoledronic acid, other ingredients in Zometa, or any other medicines. Before you start taking this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Asthma;
• Atypical femur fractures;
• Jaw or dental problems;
• Kidney disease;
• Liver disease;
• Low calcium levels

One rare but possible adverse effect of this medication is osteonecrosis of the jawbone, which can cause symptoms such as jaw pain, swelling, or infection of the gums, loose teeth, and poor oral health. Therefore, before starting Zometa, it is recommended to have a dental examination, especially if you have a history of dental problems or are undergoing invasive dental procedures. Good oral hygiene and routine dental visits should be maintained throughout treatment with this medication. While receiving zoledronic acid, try to postpone invasive dental procedures, such as tooth extractions, until your zoledronic acid treatment is complete. Zometa should not be used during pregnancy. Animal studies show it can cross the placenta and cause fetal harm, specifically affecting bone development and leading to low birth weight. Because the drug stays in your bones for years, it could theoretically affect a pregnancy even after you stop treatment. Most doctors require a pregnancy test before starting it in women of childbearing age. It is generally advised to avoid breastfeeding while on Zometa. While it’s not fully known if the drug passes into human milk, the potential for it to interfere with an infant's developing bones is a significant concern. This medication is not for use in children.

Using zoledronic acid with other medications such as certain antibiotics (aminoglycosides), some anti-inflammatories (NSAIDs), or specific chemotherapy agents can strain the kidneys, increasing the risk of kidney failure. Therefore, before using this medication, it is important to inform your doctor about any other medications you are currently taking, including herbal medications and supplements, to determine if it is safe to use along with zoledronic acid. This medication is typically administered by a qualified healthcare provider. It is typically administered by slow intravenous infusion. The infusion typically lasts at least 15 minutes to protect your kidneys. Drink at least two glasses of water or other fluids shortly before the treatment to stay hydrated. Your doctor will determine the appropriate dosing schedule for you depending on your condition. You should also take daily calcium and vitamin D supplements as directed by your doctor. This medication is stored at room temperature. After mixing, it is stored in the refrigerator. Keep this medication out of the sight and reach of children and pets.
