Naloxone works by competing with opioids for specific receptor sites in the central nervous system, including the mu, kappa, and delta opiate receptors. It has its highest affinity for the mu receptor. Once administered, naloxone rapidly crosses the blood-brain barrier and binds to mu-opioid receptors. Because it possesses a higher binding affinity for these receptor sites than most full or partial opioid agonists, it physically displaces active opioid molecules like fentanyl, heroin, or morphine from the receptors.

Because it is an essentially pure antagonist, it does not have any of the morphine-like or agonist qualities that opioids have, meaning it does not cause respiratory depression or pupillary constriction when used on its own. By blocking mu-opioid receptors, allows the brainstem's respiratory centers to resume normal electrical signaling to the diaphragm, restoring normal breathing.

This medication usually begins to work within two minutes to reverse the effects of the opioids. It is important to note that its duration of action (roughly 30 to 90 minutes) is often shorter than that of the opioids being treated, which can allow the effects of the opioids to return as Naloxone leaves the body.
