What is Naloxone (Naloxone Hydrochloride) Used For?
Naloxone (Naloxone Hydrochloride) is an opioid antagonist used as an
antidote medication to partially or completely reverse life-threatening opioid toxicity and restore normal breathing during a known or suspected opioid overdose. It is primarily used for the acute treatment of central nervous system and respiratory depression caused by natural or synthetic opioids such as heroin, fentanyl, oxycodone, morphine, and methadone. This medication is also used to help healthcare providers diagnose a known or suspected acute opioid overdose. It may also be administered postoperatively to reverse excessive opioid-induced sedation after surgery. It may be used as an additional treatment to help increase blood pressure in patients suffering from septic shock. In any suspected overdose, calling emergency medical services (e.g., 911) immediately is the critical first step before or during naloxone administration.
Naloxone works by competing with opioids for specific receptor sites in the central nervous system, including the mu, kappa, and delta opiate receptors. It has its highest affinity for the mu receptor. Once administered, naloxone rapidly crosses the blood-brain barrier and binds to mu-opioid receptors. Because it possesses a higher binding affinity for these receptor sites than most full or partial opioid agonists, it physically displaces active opioid molecules like fentanyl, heroin, or morphine from the receptors.
Because it is an essentially pure antagonist, it does not have any of the morphine-like or agonist qualities that opioids have, meaning it does not cause respiratory depression or pupillary constriction when used on its own. By blocking mu-opioid receptors, allows the brainstem's respiratory centers to resume normal electrical signaling to the diaphragm, restoring normal breathing.
This medication usually begins to work within two minutes to reverse the effects of the opioids. It is important to note that its duration of action (roughly 30 to 90 minutes) is often shorter than that of the opioids being treated, which can allow the effects of the opioids to return as Naloxone leaves the body. Therefore, individuals who have received this medication must be kept under continuous medical surveillance, as repeated doses may be required as determined necessary by a qualified healthcare provider.
Naloxone reverses only opioid toxicity. It is ineffective against respiratory depression caused by non-opioid central nervous system depressants, such as benzodiazepines, xylazine, alcohol, and barbiturates.
A qualified healthcare provider must administer this medication. However, in an emergency, if administered by non-medical personnel, give it intramuscularly. For complete information about this medication, see its
product monograph.
What Side Effects Can Naloxone or Generic Naloxone Hydrochloride Cause?
Naloxone is generally safe to use, and most side effects of naloxone often manifest as symptoms of precipitated opioid withdrawal, especially in individuals who are physically dependent on opioids. These include nausea, vomiting, abdominal cramps, diarrhea, body aches (myalgia), weakness, fever, excessive sweating, runny nose (rhinorrhea), sneezing, and frequent yawning.
Other possible side effects include nervousness, restlessness, irritability, dizziness, hot flashes, flushing, tremors, and excessive crying in neonates. Inform your doctor if these mild side effects persist or worsen.
Serious side effects may include:
• Allergic reactions such as hives, skin blistering, swelling of the face, lips, tongue, or throat, or difficulty breathing or swallowing;
• Respiratory problems such as non-cardiogenic pulmonary edema with symptoms such as severe shortness of breath, wheezing, or frothy sputum, likely triggered by a massive sympathetic surge;
• Severe cardiovascular reactions such as abnormally fast heartbeat (tachycardia), cardiac arrest, arrhythmias, severe hypotension, or severe high blood pressure;
• Severe neurological problems such as seizures, coma, encephalopathy, paraesthesia (tingling sensations), and cognitive impairment;
Seek immediate medical attention if you experience any of these serious side effects after you’ve been given Naloxone. Also, inform your doctor about any other side effects you notice that are not listed here.
Warnings
Do not take Naloxone if you are allergic to naloxone hydrochloride or any of its other ingredients. Administer this medication with caution to anyone physically dependent on opioids, including newborns of dependent mothers. Naloxone can trigger acute withdrawal, which may cause high blood pressure, fast or irregular heartbeat, severe breathing difficulty, or cardiac arrest. Also use caution if you have any of the following medical conditions:
• Heart or circulation problems;
• Kidney disease;
• Liver disease;
• Seizure disorder;
It is also worth noting that reversal of buprenorphine toxicity may be incomplete, potentially requiring mechanical respiratory assistance or higher/repeated doses of naloxone. Large doses of naloxone are often required to antagonize buprenorphine because of its slow binding rate and slow dissociation from opioid receptors. Concomitant use with medications that affect blood pressure or cardiac conduction may increase the risk of severe cardiovascular complications during the sympathetic surge of rapid reversal. Before or after using this medication, inform your doctor about any other medications you are currently taking, including herbal medicines and supplements, to determine if it is safe to use along with Naloxone.
This medication should be used during pregnancy or while breastfeeding only if clearly needed, as naloxone crosses the placenta, and rapid administration can trigger fetal withdrawal, potentially causing fetal distress or preterm labor in mothers who are physically dependent on opioids. However, maternal resuscitation during an overdose takes priority, and naloxone should not be withheld. It is unknown if it passes through breast milk. Store this medication at 15°C to 25°C (59°F to 77°F), protect from freezing, and keep it away from direct light, moisture, and children's reach.
