Dostinex Information

Dostinex (Cabergoline) is a potent, long-acting dopamine receptor agonist primarily used to treat conditions associated with abnormally high levels of the hormone prolactin in the blood, also known as hyperprolactinemia. These conditions include idiopathic hyperprolactinemia, in which the cause of the elevated prolactin is unknown, and prolactin-secreting pituitary adenomas, benign tumors of the pituitary gland classified as microprolactinomas or macroprolactinomas depending on their size. By lowering prolactin levels, Dostinex helps to treat the associated symptoms of hyperprolactinemia, such as missing or irregular menstrual periods and abnormal breast milk production in both men and women.

Additionally, it may be used to inhibit breastfeeding immediately after childbirth in women who have had a stillbirth or miscarriage or to suppress breast milk production once it has already started. Beyond its primary use, cabergoline is sometimes used to help shrink growth-hormone-secreting tumors in individuals with acromegaly and to manage motor symptoms in Parkinson's disease, although other non-ergot dopamine agonists are generally preferred for the treatment of Parkinson's disease.

Cabergoline works by mimicking dopamine, the brain's natural chemical messenger that helps regulate prolactin levels. It travels to the anterior pituitary gland and binds directly to dopamine receptors located on the surface of prolactin-producing cells. Once attached, it triggers an inhibitory signal in these cells, effectively turning off prolactin production and release into the bloodstream. Dostinex Side Effects

Common side effects of this medication include drowsiness, nausea, headache, dizziness, vertigo, stomach pain, indigestion (dyspepsia), fatigue, constipation, blurred vision, breast pain, depression, sleep disturbances, vomiting, low blood pressure, and hot flushes. It can also cause severe drowsiness and, in rare instances, sudden sleep onset episodes without warning signs, so avoid driving or operating machinery until you know how the medication affects you. Contact your doctor if these mild side effects persist or worsen. You may also experience an inability to resist strong impulses like binge eating, increased sexual drive, aggressiveness, and gambling impulses, all of which could be harmful to you or others. Discuss these symptoms with your doctor, as you may need a dosage adjustment.

• Abnormal or unusual thoughts;

Dostinex Precautions

Do not use this medication if you have:
• Allergic reaction to cabergoline, ergot derivatives, or other ingredients in Dostinex;
• Current or past history of heart valve diseases or pericarditis;
• Current or past history of mental illness after birth (postpartum psychosis) or you are being treated with antipsychotics;
• Current or past history of pulmonary fibrosis, or retroperitoneal fibrotic disorders;
• Severe liver disease;
• Uncontrolled hypertension, including high blood pressure in pregnancy (preeclampsia);

This medication needs to be used with caution in individuals with certain medical conditions. Before starting this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Gastrointestinal problems such as peptic ulcer disease, stomach or intestinal bleeding;

• Kidney disease;
• Low blood pressure;
• Raynaud’s syndrome (cold hands and feet due to poor blood circulation);

Erythromycin, clarithromycin, and other macrolide antibiotics can increase the body's exposure to cabergoline by blocking its metabolism or clearance, potentially increasing the risk of adverse effects. Because cabergoline causes postural hypotension, co-administration with blood pressure-lowering agents requires cautious dose adjustments to prevent symptomatic hypotension or syncope. Therefore, before using this medication, inform your doctor of any other medications you are currently taking, including herbal medicines and supplements, to determine whether it is safe to use with cabergoline.

For women planning to conceive, cabergoline should ideally be discontinued one month before pregnancy due to its long half-life, though it is usually stopped once pregnancy is confirmed unless a large pituitary tumor requires ongoing management. While low doses do not appear to increase miscarriage or birth defect risks, monitoring remains imperative. Also, this medication should not be used in women who are breastfeeding because its primary action inherently prevents milk production by shutting down prolactin secretion. Additionally, it remains unknown whether the medication is excreted into breast milk, posing an undetermined risk to a nursing infant. Store this medication at room temperature, away from sunlight and moisture, and out of children's reach.
