Leucovorin Calcium (Calcium folinate) Information
Leucovorin calcium, which contains folinic acid, is a form of vitamin B9 used in several serious medical situations. It is commonly given as a “rescue” medication after high-dose methotrexate to protect healthy cells, to treat folate deficiency, to reduce side effects of certain chemotherapy drugs, and to improve the effect of fluorouracil (5-FU) in some cancers. It is also used to treat certain types of anemia caused by folate deficiency. Leucovorin can be taken as oral tablets or given by injection (intravenous or intramuscular), depending on the condition. After methotrexate, the dosing schedule is exact to help the body safely clear the drug.
The mechanism of action of Leucovorin involves bypassing the body’s need to activate folic acid. It provides a form of folate that the body can use directly. This helps cells make DNA and repair themselves. After methotrexate, Leucovorin replenishes folate in healthy cells, reducing toxicity. When combined with 5-FU, it stabilizes the drug and makes it more effective against cancer.
Leucovorin Side Effects
Leucovorin is generally well-tolerated, but some common side effects may occur. Commonly reported effects include nausea, vomiting, diarrhea, fatigue, and mild stomach discomfort. Some people may experience headaches, rashes, or trouble sleeping.
More serious side effects are uncommon but require immediate medical attention. Stop the medication and seek urgent care if any of the following symptoms appear:
• Severe allergic reaction, including swelling of the face, lips, or throat; difficulty breathing; hives
• Severe or persistent diarrhea
• Sudden dizziness, fainting, or a fast heartbeat
• Worsening symptoms when used with 5-FU, including mouth sores, severe abdominal pain, fever, or bleeding
Leucovorin Precautions
Do not take Leucovorin if you are allergic to folinic acid or any ingredient in the medication. Before starting treatment, inform your doctor of your full medical history. People with anaemia due to vitamin B12 deficiency should not use Leucovorin alone, as it may mask the nerve damage associated with B12 deficiency. Those with seizure disorders may need careful monitoring, as Leucovorin can interact with some seizure medications.
Tell your doctor about all medications you take, including prescription drugs, over-the-counter products, vitamins, and herbal supplements. Leucovorin may interact with methotrexate, 5-fluorouracil, trimethoprim, pyrimethamine, and several anticonvulsants. These interactions may require dose adjustments or enhanced monitoring.
Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Leucovorin is generally considered safe when medically necessary, but its use should be supervised by a doctor, especially during chemotherapy. It is unknown whether calcium folinate is excreted into breast milk. Discuss the benefits and risks of using this medication when breastfeeding with your doctor.
Take Leucovorin exactly as prescribed. After methotrexate, follow the dosing schedule carefully to ensure proper protection. When used with 5-FU, doses are usually given in a hospital or clinic. You can take the oral tablets with or without food unless told otherwise. Do not change your dose or stop treatment without medical advice. Store at room temperature, protected from heat and moisture.
