Procytox Information

Procytox (Cyclophosphamide) is a well-known chemotherapy agent that is highly versatile because it can shut down rapidly dividing cells, whether they are cancerous or overactive immune cells. It is used, usually in combination with other chemotherapy, for the treatment of various blood-related cancers, including Hodgkin’s and non-Hodgkin lymphomas, multiple myeloma, and several leukemias such as chronic lymphocytic leukemia and acute lymphocytic leukemia. Furthermore, it is effective against solid tumors, notably breast and ovarian cancers, as well as pediatric cancers such as neuroblastoma and retinoblastoma. Aside from its role in cancer management, it is also useful in the treatment of life-threatening autoimmune conditions, most especially systemic lupus erythematosus. This is its most significant non-cancer use. It’s the gold standard therapy for lupus nephritis (when lupus attacks the kidneys) and CNS lupus (when it affects the brain), especially during a severe flare, to prevent permanent organ damage. It is also used in other autoimmune conditions such as vasculitis and systemic sclerosis.

Beyond lupus, Procytox is used for specific types of kidney inflammation where the immune system is the culprit. It is used in the treatment of nephrotic syndrome in both children and adults, especially in cases that are not responsive to steroids. It is used to prevent progression to kidney failure. Furthermore, this medication is useful in transplant medicine, where it's used to ablate the bone marrow before stem cell transplantation or to prevent graft-versus-host disease. Before a patient receives a bone marrow or stem cell transplant, their immune system must be suppressed to make room for the new cells and prevent rejection. It is a staple therapy in this case.

Cyclophosphamide has also been found useful for neurological conditions mediated by immune hyperactivity, such as multiple sclerosis. It isn't a standard daily multiple sclerosis therapy. Still, induction therapy with cyclophosphamide is sometimes used for patients with highly active, aggressive relapsing-remitting multiple sclerosis who aren't responding to newer biological therapies.

The active ingredient in Procytox, cyclophosphamide, is an alkylating agent. As a prodrug, it undergoes hepatic conversion into active metabolites that function by attaching alkyl groups to DNA, creating cross-links that physically obstruct the double helix from uncoiling. In cancer treatment, this mechanism appears to induce programmed cell death in rapidly proliferating cancer cells, effectively halting tumor progression. When used for autoimmune diseases, it likely helps by depleting overactive lymphocytes that drive tissue inflammation, thereby preventing permanent organ damage or fibrosis. While highly effective, its systemic impact is significant, often requiring careful monitoring of blood counts and bladder health because of its non-selective DNA-damaging effects. This dual role as both a cytotoxic chemotherapy and a potent immunosuppressant makes it a cornerstone in managing life-threatening or refractory diseases that have failed to respond to milder therapeutic interventions.

Procytox Side Effects

Like other medications, cyclophosphamide is associated with some side effects. Common mild side effects of this medication include diarrhea, fatigue, feeling generally unwell, headache, loss of appetite, nausea, temporary hair loss (returns after treatment ends, although texture or colour may change), and vomiting. Pain, swelling, or redness at the injection site of the injectable form may also occur. Contact your doctor if these mild side effects persist or worsen.

Stop taking this medicine and contact your doctor immediately if you experience any serious side effects, including the following:
• Allergic reactions such as breathing difficulties, body itching, body rash, skin blistering, swelling of the face, mouth, tongue, lips, or throat;
• Anemia (low red blood cells) with symptoms like dizziness, pale skin, unusual tiredness;
• Heart failure with symptoms like shortness of breath, fatigue, swelling in legs, ankles, and feet;
• Infection with symptoms like fever or chills, cough, shortness of breath, prolonged dizziness, headache;
• Kidney problems with symptoms such as increased urination at night, decreased urine production;
• Liver problems with symptoms like nausea, vomiting, diarrhea, loss of appetite, weight loss, and yellowing of the skin or whites of the eyes;
• Lung inflammation with symptoms like darkening of the lips or fingernails, persistent cough, and burning sensation in the chest;
• Seizures;
• Urinary problems such as blood in urine, painful urination

Procytox Precautions

Do not take this medication if you are allergic to cyclophosphamide, other ingredients in Procytox, or any other medicines. Before you start taking this medication, ensure that you inform your doctor about any prior or current medical conditions, including the following:
• Active or chronic infection;
• Blood cell disorders such as low red or white blood cells, including severe suppression of bone marrow;
• Electrolyte imbalances;
• Heart disease;
• Kidney disease;
• Liver disease;
• Lung disease;
• Surgery to remove the adrenal glands;
• Urinary tract problems

You should generally avoid live vaccines (such as the MMR or yellow fever vaccines) while on this medication. Since your immune system may be suppressed, the vaccine could actually cause the disease it’s meant to prevent. This medication may prevent wound healing. Therefore, inform your doctor that you are taking this medication if you are planning surgery. Procytox is highly teratogenic, meaning it is known to cause serious congenital disabilities or fetal death. It is strictly contraindicated during pregnancy. A negative pregnancy test is typically required before starting, and patients must use highly effective contraception during treatment and for at least 12 months after the final dose (men are often advised to wait at least 6 months). Even handling the tablets can be risky; pregnant women are often advised not even to touch broken pills or breathe in the dust. Breastfeeding is not recommended while taking cyclophosphamide as it can be excreted into breast milk in significant amounts. There have been reports of infants developing neutropenia (dangerously low white blood cell counts) after being breastfed by mothers on this medication. Generally, a wait period of at least one week after the last dose is recommended before resuming breastfeeding. There is a very high likelihood of permanent infertility in both men and women. It can cause premature menopause in women and a permanent stop in sperm production (azoospermia) in men. Discuss these risks with your doctor. Procytox is not recommended for use in children under 16 years of age. Cyclophosphamide might impair your ability to drive or operate machinery. Avoid engaging in such activities until you can evaluate how the medication affects you.

Drugs that speed up or slow down liver enzymes (such as rifampin or phenytoin) can drastically alter the amount of cyclophosphamide converted into its toxic active form. Likewise, allopurinol can increase cyclophosphamide's bone marrow toxicity. Therefore, before using this medication, inform your doctor about any other medications you are currently taking, including herbal medicines and supplements, to determine whether it is safe to use with Procytox. This medication is available as tablets, intended for oral use only. It is also available in powder form, which should be reconstituted into injection solutions by your doctor or nurse. Your doctor will determine the appropriate dose, formulation, and duration of treatment for you. It is important to drink plenty of fluids (at least 2 litres per day) while taking this medication to prevent side effects.
